血清骨保护素、骨桥蛋白、骨钙素水平与冠心病患者冠状动脉狭窄程度的关系 被引量:1

Correlation of serum osteoprotegerin,osteopontin and osteocalcin with the degree of coronary artery stenosis in patients with coronary artery disease

摘要 目的探讨冠心病患者血清骨保护素(OPG)、骨桥蛋白(OPN)、骨钙素等骨因子水平与冠心病患者冠状动脉狭窄程度的关系。方法收集2019年1月至2019年12月锦州市中心医院接收的110例冠心病患者资料,根据冠状动脉造影显示的血管狭窄程度分为两组,即对照组(血管狭窄程度<50%)42例与稳定型冠心病患者(血管狭窄程度≥50%)68例,其中稳定型冠心病患者再根据Gensini评分分为低分组与高分组各34例。收集各组的一般资料,并检测各组OPG、OPN、骨钙素等骨因子的表达水平,分析其与冠心病患者冠状动脉狭窄程度的关系。结果与对照组相比,稳定型冠心病患者的OPG、OPN水平均显著升高,而骨钙素明显降低(F=15.263、130.298、158.671,P<0.05);高分组OPG、OPN水平高于低分组,骨钙素低于低分组,差异有统计学意义(t=7.152、6.884、19.641,P<0.05)。Pearson相关分析显示,OPG、OPN与Gensini评分呈正相关,骨钙素与Gensini评分呈负相关(r=0.624、0.538、-0.637,P<0.05)。逐步回归分析发现OPG、OPN、骨钙素均是冠状动脉狭窄程度的相关影响因素[OR(95%CI)=10.268(0.357~12.643)、2.369(0.326~3.674)、3.712(0.372~4.628),P<0.05]。结论冠心病患者冠状动脉狭窄程度越严重,其体内OPG、OPN的表达就越高,骨钙素表达越低,其可能成为冠状动脉狭窄严重程度的可靠预测指标。 Objective To explore the correlation of serum osteoprotegerin(OPG),osteopontin(OPN)and osteocalcin with the degree of coronary artery stenosis in patients with coronary heart disease.Methods 100 patients with coronary heart disease admitted to the Central Hospital of Jinzhou City from January 2019 to December 2019 were collected,and were divided into control group(degree of vascular stenosis<50%)(n=42)and stable coronary heart disease group(degree of vascular stenosis≥50%)(n=68)according to the degree of coronary artery stenosis based on coronary angiography.The stable coronary heart disease group was sub-classified as low-score group and high-score group according to Gensini scores,34 cases in each group.General information for each group was collected,the expression levels of OPG,OPN and osteocalcin were detected,and the correlation of OPG,OPN and osteocalcin with the degree of coranary artery stenosis was analyzed.Results Compared with the control group,OPG and OPN levels in the stable coronary heart disease group were obviously increased while osteocalcin levels were reduced(F=15.263,130.298,158.671;P<0.05).Compared with the low-score group,OPG and OPN levels in the high-score group were sharply increased while osteocalcin levels were reduced(t=7.152,6.884,19.641;P<0.05).Pearson analysis indicated that OPG and OPN levels were positively correlated with Gensini scores,while osteocalcin levels were negatively correlated with Gensini scores(r=0.624,0.538,-0.637;P<0.05).The stepwise regression analysis indicated that OPG,OPN and osteocalcin were the influence factors of coronary artery stenosis degree[OR(95%CI)=10.268(0.357-12.643),2.369(0.326-3.674),3.712(0.372-4.628);P<0.05].Conclusion The more severe the degree of coronary artery stenosis in patients with coronary heart disease,the higher the expressions of osteoprotegerin and osteopontin and the lower the expression of osteocalcin expressions,which may be a reliable predictor of the severity of coronary artery stenosis.
