中药饮片不合理处方的点评分析

Comments on unreasonable prescriptions of Chinese herb pieces

目的探究中药饮片处方的用药合理性以及规范性,查找相应的验证依据并针对不合理处提出相关解决措施。方法针对山东大学齐鲁医院2019年3月-2020年5月期间的1 400张处方进行分析,每月100张处方,对处方的不合理处进行统计。结果经过点评分析,不合理处方有205张,不合理率为14.7%。其中包括未按要求标注药物调剂和煎煮等特殊要求、处方未按照君臣佐使顺序排列、用量不适宜、适应证不适宜、有配伍禁忌或是不良相互作用、药品给药途径不适宜、重复给药、处方金额过大、味数过多等。结论各医疗机构应当重视对中药师以及临床医师的培养,提高中药点评的水平,明确相关点评监督制度,落实责任到个人,进而保证中药饮片不合理处方逐渐减少,提升人们的用药安全性。

Objective Explore the rationality and standardization of Chinese herb pieces prescription,find the corresponding verification basis,and put forward relevant solutions for the unreasonable.Methods The 1 400 prescriptions of Qilu Hospital of Shandong University from March 2019 to May 2020 were analyzed,100 prescriptions per month,and the unreasonable prescriptions were counted.Results After review and analysis,there were 205 unreasonable prescriptions,and the unreasonable rate was 14.7%.Including those failing to mark the special requirements for drug dispensing and decocting as required,the prescriptions are not arranged in the order of kings,ministers and envoys,inappropriate dosage,inappropriate indications,incompatibility or adverse interaction,inappropriate route of drug administration,repeated administration,the prescription amount is too large,and the number of medicinal materials contained in a single prescription is excessive,etc.Conclusion All medical institutions should pay attention to the training of Chinese pharmacists and clinicians,improve the level of traditional Chinese medicine comments,clarify the relevant comment supervision system,and implement the responsibility to individuals,so as to ensure that the unreasonable prescriptions of Chinese herb pieces are gradually reduced and improve people’s drug safety.
