华氏巨球蛋白血症伴肾淀粉样变性及淋巴瘤浸润1例 被引量：1

Amyloidosis and lymphomatous infiltration of the kidneys associated with Waldenstrom macroglobulinemia:one case report

摘要 报告1例华氏巨球蛋白血症(Waldenstrom macroglobulinemia)伴轻重链肾淀粉样变性(heavy-and light-amyloidosis,AHL)及肾间质淋巴瘤浸润的罕见病例。中年男性患者,贫血伴IgM-κ型单克隆免疫球蛋白血症,骨髓浆细胞样淋巴细胞浸润,肾脏表现为肾病综合征伴肾功能不全,肾活检示肾小球、肾间质及细动脉可见均质无结构物质沉积,刚果红染色阳性,淀粉样沉积IgM及κ轻链染色呈强阳性,IgG、IgA、λ轻链阴性,电镜见直径8~12 nm僵硬无分支排列紊乱的纤维丝,同时肾间质见灶性聚集的小淋巴细胞及浆细胞样淋巴细胞浸润,经环磷酰胺+长春新碱+泼尼松(COP)方案治疗有效。
