多层螺旋CT联合MRI诊断早期股骨头坏死的价值分析

摘要 目的探讨多层螺旋CT联合MRI诊断早期股骨头坏死的效果,为临床早期诊断与治疗提供依据。方法回顾性分析凤阳县中医院2018年1月至2021年6月收治的60例疑似早期股骨头坏死患者的临床资料,均实施多层螺旋CT、MRI检查,以手术病理检查结果为金标准,比较多层螺旋CT、MRI单独与联合检查对早期股骨头坏死各分期的阳性检出率、诊断结果、诊断价值及髋关节积液检出率。结果多层螺旋CT联合MRI的阳性检出率显著高于多层螺旋CT单独检查;经手术病理检查结果显示,60例疑似早期股骨头坏死患者中阳性48例,阴性12例。多层螺旋CT检查诊断阳性37例,阴性23例,其中仅28例为真实早期骨头坏死患者;MRI检查诊断阳性40例,阴性20例,其中仅35例为真实早期骨头坏死者;联合检查诊断阳性46例,阴性14例,其中仅44例为真实早期骨头坏死者;多层螺旋CT联合MRI检查的特异度、灵敏度、准确度、阴性预测值均显著高于多层螺旋CT、MRI单独检查(均P<0.05),联合检查阳性检出率与多层螺旋CT、MRI单独检查组间比较,差异均无统计学意义(均P>0.05)。多螺旋CT联合MRI对髋关节积液检出率显著高于多层螺旋CT、MRI单独检查(P<0.05)。结论多层螺旋CT联合MRI检查对早期股骨头坏死有很高的诊断价值,相较于多层螺旋CT、MRI单独检测,其对髋关节积液检出率较高,在今后的临床影像学检查中可运用联合检测的方式对患者进行检测,减少或避免漏诊、误诊情况的发生。
