探讨无创持续气道正压通气(CPAP)联合布地奈德在新生儿呼吸窘迫综合征中的临床应用价值 被引量:3

摘要 目的探讨无创持续气道正压通气(CPAP)联合布地奈德雾化治疗新生儿呼吸窘迫综合征的临床效果。方法随机选取2020年1月—2021年6月该院收治的70例新生儿呼吸窘迫综合征患儿为研究对象,随机分为对照组和观察组,各35例。对照组给予CPAP治疗,观察组给予CPAP联合布地奈德雾化治疗。比较两组CPAP治疗时间、住院时间、并发症发生率、治疗24 h后NEWS评分、治疗2 h和24 h后动脉血气分析指标。结果观察组的CPAP治疗时间和住院时间均短于对照组,差异有统计学意义(P<0.05);观察组并发症总发生率(5.71%)低于对照组(22.86%),差异有统计学意义(χ^(2)=4.200,P<0.05);经治疗后,观察组NEWS评分低于对照组,差异有统计学意义(P<0.05),在动脉血气分析方面,治疗2 h和24 h后观察组的PaCO2低于对照组,而PaO2和pH值则高于对照组,差异有统计学意义(P<0.05)。结论CPAP联合布地奈德雾化治疗新生儿呼吸窘迫综合征可提高临床效果,同时缩短CPAP治疗时间和住院时间,减少并发症发生率,并明显改善NEWS评分和动脉血气分析指标,改善疾病预后,且具有较高安全性,值得临床推广应用。 Objective To investigate the clinical effect of noninvasive continuous positive airway pressure(CPAP)combined with budesonide nebulization in the treatment of neonatal respiratory distress syndrome.Methods 70 infants with neonatal respiratory distress syndrome admitted to the hospital from January 2020 to June 2021 were randomly selected as the research objects,and they were randomly divided into a control group and an observation group,with 35 cases in each group.The control group was given CPAP treatment,and the observation group was given CPAP combined with budesonide nebulization treatment.The CPAP treatment time,hospital stay,complication rate,NEWS score after 24 h treatment,arterial blood gas analysis indexes after 2 h and 24 h treatment were compared between the two groups.Results The CPAP treatment time and hospital stay of the observation group were shorter than those of the control group,the difference was statistically significant(P<0.05).The total incidence of complications in the observation group(5.71%)was lower than that of the control group(22.86%),the difference was statistically significant(χ^(2)=4.200,P<0.05).After treatment,the NEWS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).In terms of arterial blood gas analysis,PaCO2 of the observation group was lower than that of the control group after 2 h and 24 h treatment,while PaO2 and pH values were higher than the control group,the difference was statistically significant(P<0.05).Conclusion CPAP combined with budesonide nebulization in the treatment of neonatal respiratory distress syndrome can improve the clinical effect,while shortening the CPAP treatment time and hospital stay,and reducing the incidence of complications.And significantly improve the NEWS score,arterial blood gas analysis indicators,and improve the prognosis of the disease.It has high safety and is worthy of clinical application.
