小儿肺炎支原体肺炎的临床分析 被引量:2

摘要 目的研究探讨2019年该院小儿肺炎支原体肺炎的临床特点。方法回顾性选取2019年5—7月该院儿科呼吸组收治的符合肺炎和肺炎支原体感染诊断标准的患儿170例,同时对比2018年5—7月同期收治的小儿肺炎支原体患儿88例,比较两年收治的肺炎支原体感染患儿的基本情况、血液检测及影像学检查结果、治疗及转归情况。结果2019年院外发热时间较2018年延长,2019年平均住院时间较2018年延长,2019年发病年龄大于2018年,差异有统计学意义(P<0.05)。白细胞、中性粒细胞、血小板、CRP、单核细胞等均值比较,差异无统计学意义(P>0.05)。2019年抗体滴度对数转换(2.55±0.59)明显高于2018年(2.28±0.44),差异有统计学意义(t=3.782,P<0.05)。2018年88例合并细菌感染5例,2019年170例有41例合并细菌感染,差异有统计学意义(P<0.05)。结论小儿肺炎支原体肺炎的表现复杂,无论病原体是否改变,根据治疗及转归情况推测与阿奇霉素耐药率逐年上升有关。 Objective To study and explore the clinical characteristics of Mycoplasma pneumoniae pneumonia in children in the hospital in 2019.Methods From May 2019 to July 2019,170 children admitted to the pediatric respiratory group of the hospital who met the diagnostic criteria for pneumonia and Mycoplasma pneumoniae infection were retrospective selected.At the same time,the 88 cases of Mycoplasma pneumonia in children admitted in the same period in 2018 were compared.The basic information,blood test and imaging examination results,treatment and outcome of children with Mycoplasma pneumoniae infection admitted for two years were compared.Results The duration of out-of-hospital fever in 2019 were longer than in 2018,the average length of hospital stay in 2019 was longer than that in 2018,and the age of onset in 2019 was greater than that in 2018,the difference was statistically significant(P<0.05).There was no statistically significant difference in the mean values of white blood cells,neutrophils,platelets,CRP,monocytes(P>0.05).The logarithmic conversion of antibody titer in 2019(2.55±0.59)was significantly higher than that in 2018 of(2.28±0.44),and the difference was statistically significant(t=3.782,P<0.05).In 2018,there were 88 cases of combined bacterial infection in 5 cases,170 cases in 2019,41 cases of combined bacterial infection,the difference was statistically significant(P<0.05).Conclusion The manifestations of Mycoplasma pneumoniae pneumonia in children are complicated.Regardless of whether the pathogen changes,it is inferred that it is related to the increasing rate of azithromycin resistance based on the treatment and outcome.
