摘要
目的研究不同位置高血压脑出血患者血清S100β蛋白的表达特点以及临床意义。方法选取2018年1月—2020年1月该科收治的出血量5~15 mL高血压脑出血患者为研究对象,按出血位置不同分组,丘脑出血(n=40)、基底节出血(n=50)、脑叶出血(n=30)、小脑出血(n=30)。每例患者均留存发病后12 h、24 h、48 h、72 h、7 d血清,使用金磁生物技术公司提供的免疫磁定量监测试剂盒测定血清S100β水平,对4组患者的平均血清SS100β蛋白表达量和平均出血量进行统计,然后分析12 h、24 h、48 h、72 h、7 d血清S100β蛋白表达量和患者对应时间下的Barthel指数(日常生活活动功能评分)的关系,进而研究其不同位置脑出血患者的预后效果。结果丘脑出血患者的平均出血量为(10.46±1.05)mL,基底节出血患者的平均出血量为(10.54±1.12)mL,脑叶出血患者的平均出血量为(10.32±1.13)mL,小脑出血患者的平均出血量为(10.36±1.16)mL,4组患者的平均出血量比较,差异无统计学意义(P>0.05);丘脑出血患者和基底节出血患者的不同时间下的血清S100β蛋白表达量要高于脑叶出血患者和小脑出血患者,差异有统计学意义(P<0.05)。丘脑出血患者和基底节出血患者的Barthel指数低于脑叶出血患者和小脑出血患者,差异有统计学意义(P<0.05)。结论高血压脑出血患者于基底节以及丘脑出血的血清S100β蛋白的表达量高于脑叶出血以及小脑出血患者,因此,临床中需要格外重视基底节出血以及丘脑出血患者,可以通过检测血清S100β蛋白的表达量,对患者的出血严重程度以及预后效果进行评估。
Objective To study the expression characteristics and clinical significance of serum S100βprotein in patients with hypertensive intracerebral hemorrhage in different locations.Methods Patients with hypertensive intracerebral hemorrhage with a bleeding volume of 5-15 mL treated in the department from January 2018 to January 2020 were selected as the research objects.Grouped according to the location of hemorrhage,thalamic hemorrhage(n=40),basal ganglia hemorrhage(n=50),cerebral lobe hemorrhage(n=30),cerebellar hemorrhage(n=30).Each patient retained serum 12 h,24 h,48 h,72 h,and 7 d after the onset of the disease,and measured the serum S100βlevel using the immunomagnetic quantitative monitoring kit provided by Jinmag Biotech.The average serum S100βprotein expression and average bleeding volume of the four groups of patients were counted.Then analyze the relationship between 12 h,24 h,48 h,72 h,7 d serum S100βprotein expression and the Barthel index(activity of daily living function score)of patients at the corresponding time,and then study the prognostic effect of patients with cerebral hemorrhage in different locations.Results The average blood loss of patients with thalamic hemorrhage was(10.46±1.05)mL,the average blood loss of patients with basal ganglia hemorrhage was(10.54±1.12)mL,the average blood loss of patients with cerebral lobe hemorrhage was(10.32±1.13)mL,the average bleeding volume of patients with cerebellar hemorrhage was(10.36±1.16)mL,there was no statistically significant difference in the average bleeding volume of the four groups of patients(P>0.05).The serum S100βprotein expression in patients with thalamic hemorrhage and basal ganglia hemorrhage at different times was higher than that in patients with cerebral lobe hemorrhage and cerebellar hemorrhage,and the difference was statistically significant(P<0.05).The Barthel index of patients with thalamic hemorrhage and basal ganglia hemorrhage was lower than that of patients with cerebral lobe hemorrhage and cerebellar hemorrhage,and the difference was statistically significant(P<0.05).Conclusion The expression of serum S100βprotein in patients with hypertensive intracerebral hemorrhage in basal ganglia and thalamic hemorrhage is higher than that in patients with lobar hemorrhage and cerebellar hemorrhage.Therefore,it is necessary to pay special attention to patients with basal ganglia hemorrhage and thalamic hemorrhage in clinical practice.By detecting the expression of serum S100βprotein,the severity of hemorrhage and the prognostic effect of patients can be evaluated.
