老年胫骨内侧平台骨折患者的治疗中采用膝关节镜辅助微创手术对患者预后效果的影响

The Effect of Knee Arthroscopy-assisted Minimally Invasive Surgery in the Treatment of Elderly Patients with Medial Tibial Plateau Fractures on the Prognosis of Patients

目的研究膝关节镜辅助微创手术治疗老年胫骨内侧平台骨折(FTP)患者对其预后效果的影响。方法选取2019年3月—2021年3月该院收治的123例胫骨内侧平台骨折老年患者为研究对象,按随机数表法分为对照组(61例)和观察组(62例),分别行传统切开复位内固定术、膝关节镜辅助微创术,对比两组治疗效果、围术期指标、并发症及术前后膝关节功能(KSS)评分、疼痛程度(VAS)评分。结果治疗优良率比较,观察组(96.77%)高于对照组(83.60%),差异有统计学意义(χ^(2)=6.055,P<0.05);与对照组比较,观察组手术时间、骨折愈合时间及住院时间均更短,切口长度更短,术中出血量、术后引流量均更少,差异有统计学意义(t=8.534、6.769、18.267、18.661、10.072、19.149,P<0.05);与对照组比较,观察组术后VAS评分更低,KSS评分更高,差异有统计学意义(t=14.458、5.542,P<0.05);观察组并发症发生率(3.23%)低于对照组(19.67%),差异有统计学意义(χ^(2)=8.245,P<0.05)。结论膝关节镜辅助微创手术治疗老年FTP患者的疗效显著,可促进患者膝关节功能恢复,缩短患者骨折愈合时间,且并发症少,可保证治疗安全性。

Objective To study the effect of knee arthroscopy-assisted minimally invasive surgery on the prognosis of elderly patients with medial tibial plateau fractures(FTP).Methods 123 elderly patients with medial tibial plateau fractures admitted to the hospital from March 2019 to March 2021 were selected as the research objects.According to the random number table method,they were divided into control group(61 cases)and observation group(62 cases).Traditional open reduction and internal fixation and knee arthroscopy-assisted minimally invasive surgery were performed respectively.The treatment effects,perioperative indicators,complications,knee joint function(KSS)scores,and pain severity(VAS)scores before and after surgery were compared between the two groups.Results Compared with the excellent and good rate of treatment,the observation group(96.77%)was higher than the control group(83.60%),and the difference was statistically significant(χ^(2)=6.055,P<0.05).Compared with the control group,the observation group had shorter operation time,fracture healing time and hospital stay,shorter incision length,less intraoperative blood loss and postoperative drainage,and the difference was statistically significant(t=8.534,6.769,18.267,18.661,10.072,19.149,P<0.05).Compared with the control group,the observation group had a lower postoperative VAS score and a higher KSS score,and the difference was statistically significant(t=14.458,5.542,P<0.05).The incidence of complications in the observation group(3.23%)was lower than that in the control group(19.67%),the difference was statistically significant(χ^(2)=8.245,P<0.05).Conclusion Knee arthroscopy-assisted minimally invasive surgery is effective in the treatment of elderly FTP patients,which can promote the recovery of knee joint function and shorten the fracture healing time of patients.And there are few complications,which can ensure the safety of treatment.
