术前呼吸锻炼专项护理对肺癌患者术后肺功能指标的护理干预价值

Nursing Intervention Value of Special Nursing of Preoperative Respiratory Exercise on Postoperative Lung Function Index of Patients With Lung Cancer

目的探究术前呼吸锻炼专项护理对肺癌患者术后肺功能指标的护理干预价值。方法选取2018年3月—2020年5月100例在本院接受肺癌根治手术的患者作为观察对象,将其随机分为对照组与研究组,每组各50例。在手术治疗过程中对两组实施不同的护理干预措施,其中对照组行常规护理,研究组在此基础上行术前呼吸锻炼专项护理,比较两组术前及术后7 d的肺功能情况[一秒用力呼气容积(FEV_(1))、最大肺活量(FVC)]、血气指标[动脉氧分压(PaO_(2))、动脉二氧化碳分压(PaCO_(2))]、术后并发症(肺部感染、呼吸衰竭、胸腔积液、肺不张)发生率、二次插管率。结果术后7 d,研究组的FEV_(1)、FVC水平、PaO_(2)、PaCO_(2)水平均明显优于对照组(P<0.05);研究组的术后并发症发生率(12.00%)、二次插管率(10.00%)明显低于对照组(38.00%、30.00%)(P<0.05)。结论在肺癌根治手术患者中实施术前呼吸锻炼专项护理,能有效改善其术后肺功能和血气指标,降低并发症发生率和二次插管率。

Objective To explore the nursing intervention value of preoperative respiratory exercise special nursing on pulmonary function indexes of patients with lung cancer after operation.Methods From March 2018 to May 2020,100 patients with lung cancer undergoing radical surgery in our hospital were selected as the research objects and randomly divided into two groups,namely the control group and the study group,with 50 cases in each group.The control group received routine nursing,while the study group received special nursing of preoperative respiratory exercise on the basis of routine nursing.The pulmonary function(FEV_(1),FVC),blood gas index(PaO_(2),PaCO_(2)),postoperative complications(pulmonary infection,respiratory failure)]of the two groups before and 7 days after operation were compared The incidence of pleural effusion and atelectasis,and the rate of second intubation.Results Seven days after operation,the levels of FEV_(1),FVC,PaO_(2)and PaCO_(2)in the study group were significantly better than those in the control group(P<0.05);the incidence of postoperative complications(12.00%)and the rate of secondary intubation(10.00%)in the study group were significantly lower than those in the control group(38.00%,30.00%)(P<0.05).Conclusion The implementation of preoperative respiratory exercise nursing in patients with lung cancer radical operation can effectively improve the postoperative pulmonary function and blood gas index,reduce the incidence of complications and the rate of secondary intubation.
