摘要
台州市护理学会针对脑卒中出院患者的居家营养管理问题开展了多维联动的探索与实践,文章就此从建立基于脑卒中患者营养风险等级的居家营养管理方案、建立脑卒中患者从医院到家庭的无缝隙营养管理组织架构与人才体系、搭建基于居家护理信息化服务平台和互联网医院平台的"互联网+护理服务"营养管理路径等方面进行了总结,以期为国内各地区开展卒中患者居家营养管理工作提供新思路。
Taizhou nursing society has carried out multi-dimensional exploration and practice on home-based nutrition management of discharged stroke patients. This paper summarizes how to establish a home-based nutrition management scheme based on the nutritional risk level of stroke patients, how to establish the organizational structure and talent system of seamless nutrition management for stroke patients from hospital to family, and how to construct nutrition management path of “Internet plus nursing service” based on home-based care information service platform and internet hospital platform, in order to provide new ideas for the practice of home-based nutrition management of stroke patients in various regions of China.
