摘要
目的分析四川省某医院乙型肝炎(乙肝)表面抗原(HBsAg)阳性孕妇乙肝e抗原(HBeAg)水平与乙肝病毒(HBV)DNA载量的相关性。方法对2014-2020年该医院就诊孕妇筛查血清HBsAg,对HBsAg阳性者检测血清乙肝e抗原(HBeAg)、乙肝e抗体(HBeAb)、乙肝核心抗体(HBcAb)和HBV DNA载量,分析HBeAg水平与HBV DNA载量的相关性。结果2014-2020年该医院就诊孕妇HBsAg总阳性率为7.33%(7432/101354);在HBsAg阳性孕妇中,HBeAb和HBcAb阳性占68.12%,HBeAg和HBcAb阳性占21.72%,HBcAb阳性占8.17%。在HBeAg浓度(IU/mL)<0.6、0.6-10.0、10.1-100.0和>100.0的HBsAg阳性孕妇中,HBV DNA载量<500IU/mL至>100.0IU/mL的比例分别从69.67%、36.28%、2.71%和0.00%下降或上升至0.00%、8.37%、16.28%和30.14%(趋势性检验:R^(2)=0.853,P=0.003;R^(2)=0.085,P=0.482;R^(2)=0.677,P=0.012;R^(2)=0.938,P<0.001)。结论本研究就诊孕妇HBsAg阳性率较高,HBsAg阳性孕妇HBV DNA载量随着HBeAg浓度增加而升高;需加强孕妇血清HBV标志物筛查和诊疗管理。
Objective To determine the correlation between hepatitis B e antigen(HBeAg)level and hepatitis B virus(HBV)DNA load among hepatitis B surface antigen(HBsAg)positive pregnant women in a hospital of Sichuan province.Methods We screened pregnant women who visited the hospital during 2014-2020 for serum HBsAg,and measured HBeAg,hepatitis B e antibody(HBeAb),hepatitis B core antibody(HBcAb),and HBV DNA load among the HBsAg positive pregnant women.We analyzed the correlation of the level of HBeAg with the HBV DNA load.Results The overall prevalence of HBsAg was 7.33%(7432/101354).Among the HBsAg positive pregnant women,68.12%were positive to HBeAb and HBcAb;21.72%were positive to HBeAg and HBcAb;and 8.17%were positive to HBcAb.Among HBsAg positive pregnant women with HBeAg concentrations being<0.6,0.6-10.0,10.1-100.0,and>100.0 IU/mL,proportions for HBV DNA loads from<500 IU/mL to>100.0 IU/mL changed from 69.67%,36.28%,2.71%,and 0.00%to 0.00%,8.37%,16.28%,and 30.14%,respectively(trend test:R^(2)=0.853,P=0.003;R^(2)=0.085,P=0.482;R^(2)=0.677,P=0.012;R^(2)=0.938,P<0.001).Conclusions The seroprevalence of HBsAg was high among pregnant women in the study,and the HBV DNA load in HBsAg positive pregnant women increased with increasing HBeAg concentration.We should strengthen screening and management of HBV serum markers in pregnant women.
