小柴胡汤合透脓败酱散加减治疗化脓性中耳炎疗效观察

Curative effect of xiao chaihu decoction combined with modified tounong baijiang powder on suppurative otitis media

目的探讨小柴胡汤合透脓败酱散加减治疗化脓性中耳炎疗效。方法选取本院2019年1月~2020年10月期间收治的88例化脓性中耳炎患者为研究对象,按照随机数表法分为两组,对照组和治疗组分别为43例、45例,对照组给予西医抗生素治疗,治疗组在上述基础给予小柴胡汤合透脓败酱散加减治疗,连续治疗14d;比较两组患者临床疗效、复发率、临床症状恢复时间,以及炎症因子水平变化。结果治疗组临床有效率(93.33%)明显高于对照组(76.19%),治疗组复发率(2.22%)低于对照组(14.29%),差异有显著性(P<0.05);治疗组耳痛缓解时间、听力恢复时间、中耳粘膜消肿时间较对照组均明显缩短,差异有显著性(P<0.05);治疗前,两组患者炎症因子指标包括血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(THF-α)均无明显差异(P>0.05),治疗后,上述指标均下降,且治疗组下降幅度较对照组更加明显,差异有显著性(P<0.05)。结论小柴胡汤合透脓败酱散加减治疗化脓性中耳炎临床疗效满意,可降低复发率,缩短临床症状恢复时间,以及降低炎症因子指标,值得临床进一步研究和推广。

Objective To explore the curative effect of Xiao Chaihu Decoction combined with modified Tounong Baijiang Powder on suppurative otitis media.Methods A total of 88 patients with suppurative otitis media admitted to the hospital from January 2019 to October 2020 were enrolled as the research objects.According to random number table method,they were divided into control group(43 cases)and treatment group(45 cases).The control group was treated with antibiotics,while treatment group was additionally treated with Xiao Chaihu Decoction and modified Tounong Baijiang Powder for 14d.The clinical curative effect,recurrence rate,recovery time of clinical symptoms,and changes in levels of inflammatory factors were compared between the two groups.Results The clinical response rate of treatment group was significantly higher than that of control group(93.33%vs 76.19%),and recurrence rate was lower than that of control group(2.22%vs 14.29%)(P<0.05).The relief time of earache,hearing recovery time and detumescence time of middle ear mucosa in treatment group were significantly shorter than those in control group(P<0.05).Before treatment,the difference in inflammatory factors[serum interleukin-6(IL-6),tumor necrosis factorα(THF-α)]between the two groups was not statistically significant(P>0.05).After treatment,the above two indexes in both groups were decreased,whose decrease was more significant in treatment group than control group(P<0.05).Conclusion The clinical curative effect of Xiao Chaihu Decoction combined with modified Tounong Baijiang Powder is significant on suppurative otitis media,which can reduce recurrence rate,shorten recovery time of clinical symptoms,and reduce inflammatory factors.
