富血小板血浆技术联合伤口治疗师主导的个案管理在手足挤压伤患者中的应用

Application of platelet-rich plasma technology combined with wound therapist-led case management in patients with hand and foot crush injuries

目的 探讨富血小板血浆(PRP)技术联合伤口治疗师主导的个案管理在手足挤压伤患者中的应用效果。方法 选取2019年1月—2021年1月在伤口门诊治疗的手足挤压伤患者56例为研究对象,按照组间基本资料均衡可比的原则分为观察组和对照组,每组28例。两组患者均给予PRP技术治疗,对照组给予常规健康宣教,观察组在对照组基础上实施以伤口治疗师为主导的个案管理。比较两组患者压力性损伤愈合评分量表(PUSH)评分、平均愈合时间、换药依从性及护理满意度。结果 观察组患者平均愈合时间短于对照组,差异有统计学意义(P<0.05)。观察组患者护理满意度高于对照组,差异有统计学意义(P<0.05)。观察组换药依从高于对照组,差异有统计学意义(P<0.05)。观察组第8周伤口愈合例数高于对照组,差异有统计学意义(P<0.05)。两组患者第1、2、4、6周伤口随着时间延长PUSH评分逐渐下降,不同时间点比较差异有统计学意义(P时间<0.05);组间比较差异有统计学意义(P组间<0.05);组间与不同时间的交互比较差异有统计学意义(P交互<0.05)。结论 手足挤压伤患者实施PRP技术联合以伤口治疗师主导的个案管理,可有效促进伤口愈合,缩短伤口愈合时间,提高护理服务满意度。

Objective To explore the application effect of platelet-rich plasma(PRP) technology combined with wound therapist-led case management in patients with hand and foot crush injuries.Methods To select 56 patients with crush injuries of the hands and feet who were treated in the wound clinic from January 2019 to January 2021 as the study objects,and they were divided into an observation group and a control group according to the principle of balance and comparability of the basic data between the groups,with 28 cases in each group.The both groups were treated with PRP technology,the control group was given routine health education,and the observation group was given case management led by wound therapists on the basis of the control group to compare the pressure ulcer scale for healing(PUSH) score,average healing time,dressing change compliance and nursing satisfaction between the two groups.Results The average healing time of patients in the observation group was shorter than that in the control group,and the dif ference was statistically signifi cant(P<0.05).The nursing satisfaction of patients in the observation group was higher than that in the control group,and the dif ference was statistically significant(P<0.05).The compliance of dressing change in the observation group was higher than that in the control group,and the dif ference was statistically signifi cant(P<0.05).The number of wound healing cases in the observation group was higher than that in the control group in the 8 th week,and the difference was statistically significant(P<0.05).In the first,second,fourth,and sixth weeks,the wound PUSH scores of the two groups decreased gradually with time,and the dif ferences were statistically signifi cant at dif ferent time points(P<0.05);the dif ferences between the groups were statistically signifi cant(P<0.05);the interaction between groups and different time differences were statistically significant(P<0.05).Conclusion The PRP technique combined with wound therapist-led case management can ef fectively promote wound healing,shorten wound healing time,and improve nursing service satisfaction in patients with crush injuries of hands and feet.
