人绒毛膜促性腺激素与妊娠期甲状腺功能的相关性研究

Study on the relationship between human chorionic gonadotropin and thyroid function during pregnancy

目的:探讨人绒毛膜促性腺激素(hCG)与妊娠期甲状腺功能的相关性。方法:选择2020年1月—2021年5月收治的妊娠期甲状腺功能异常孕妇90例为研究对象。根据妊娠阶段分为妊娠早期(1~12周)、妊娠中期(13~27周)、妊娠晚期(28~40周),每期30例。检测不同妊娠阶段妇女hCG、促甲状腺激素(TSH)、血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、抗甲状腺球蛋白抗体(ATG)、抗甲状腺过氧化酶抗体(TPOAb)水平,分析hCG与甲状腺功能的相关性。结果:孕早期FT3和FT4水平均显著低于孕中期和孕晚期,随着妊娠时间的推进,FT3水平和FT4水平呈升高趋势,TSH水平呈降低趋势,差异有统计学意义(P<0.05)。妊娠早期FT3,FT4水平与hCG变化呈正相关(P<0.05),TSH水平与hCG变化呈负相关(P<0.05);妊娠中晚期FT3,FT4,TSH与hCG变化无相关性(P>0.05)。结论:hCG可影响甲状腺功能,孕期监测甲状腺功能,可有效预防和改善不良妊娠结局。

Objective:To explore the relationship between human chorionic gonadotropin and thyroid function during pregnancy.Methods:A total of 90 patients admitted in Hongdu Hospital of Traditional Chinese Medicine Affiliated to Jiangxi University,according to the pregnancy period,this group of patients were divided into early pregnancy(1~12 weeks),middle pregnancy(13~27 weeks)and late pregnancy(28~40 weeks)with 30 cases in each group.The human chorionic gonadotropin(hCG),thyroid-stimulating hormone(TSH),serum free triiodothyronine(FT3),free thyroxine(FT4),anti-thyroglobulin antibody(ATG),Anti-thyroid peroxidase antibody(TPOAb)level were detected,to analyze the correlation between human chorionic gonadotropin(hCG)and thyroid function.Results:Then the levels of FT3 and FT4 in the first trimester were lower than the second trimester and the third trimester.With the advancement of pregnancy,the FT3 and FT4 levels showed an upward trend,and the TSH show a decreasing trend(P<0.05).The FT3 and FT4 levels in the early pregnancy were positively correlated with hCG changes(P<0.05),and the TSH levels in the early pregnancy show changes in hCG(P<0.05),there was no correlation between FT3,FT4,TSH and hCG changes in the middle and late stages of pregnancy(P>0.05).Conclusion:Human chorionic gonadotropin can affect thyroid function.Monitoring thyroid function during pregnancy can effectively prevent and improve adverse pregnancy outcomes.
