负压引流术联合神经监护在老年骨科合并高血压患者中的应用及护理效果

Application and nursing effect of negative pressure drainage combined with nerve monitoring in elderly orthopedic patients with hypertension

目的:分析负压封闭引流术(VSD)联合神经监护在老年骨科合并高血压患者中的护理效果。方法:选取2019年6月—2020年10月收治的合并高血压的骨外科疾病患者76例,采用随机表格法分为观察组和对照组,每组38例。对照组采用常规护理模式,观察组采用负压引流技术联合神经监护。观察两组患者护理干预效果。结果:两组护理干预前血压指标比较,差异无统计学意义(P>0.05),护理干预后两组血压均有不同程度降低,且观察组低于对照组(P<0.01)。护理干预后观察组疼痛程度随着时间的推移不断降低,且护理干预后各时间点评分均低于对照组(P<0.01)。两组干预后7 d,14 d创面渗出液白细胞介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)水平均不同程度降低,且观察组均低于对照组(P<0.01)。结论:负压封闭引流技术联合神经监护可有效控制患者血压,降低感染风险,促进疾病恢复。

Objective:To analyze the effect of drainage technology combined with nerve monitoring in the treatment of elderly orthopedic patients with hypertension.Methods:From June 2019 to October 2020,76 patients with orthopedic diseases complicated with hypertension were selected and divided into intervention group and control group,38 cases in each group.The control group was treated with conventional treatment and nursing mode,and the intervention group was treated with drainage technology combined with nerve monitoring.The therapeutic effects of the two groups were observed and compared.Results:There was no difference in blood pressure indexes between the two groups before treatment(P>0.05),but they were decreased in varying degrees after treatment.The diastolic blood pressure and systolic blood pressure of the intervention group were significantly lower than those of the control group(P<0.01).The pain of the intervention group decreased with time after treatment,and the scores of 1 day,2 days and 7 days after treatment were lower than those of the control group(P<0.01).The levels of interleukin-6(IL-6)and tumor necrosis facto-α(TNF-α)in wound exudate of the two groups were decreased in varying degrees on the 7th and 14th day after intervention,and the indexes of the intervention group were lower than those of the control group(P<0.01).Conclusion:drainage technology combined with nerve monitoring can effectively control the blood pressure of patients,reduce the risk of infection,and promote the recovery of disease.
