加速康复理念下胃肠外科患者术后活动量化标准研究

Research on quantification standard of postoperative activities of patients with gastrointestinal surgery under the concept of accelerated rehabilitation

目的:观察加速康复理念下胃肠外科患者术后活动量化标准的应用效果。方法:选取2018年1月—2019年6月胃肠外科收治的100例胃肠道恶性肿瘤手术患者,按病区不同分为对照组(A病区50例)和干预组(B病区50例)。对照组采用传统活动方法,干预组根据专家咨询修订的术后活动量化标准进行活动。对比两组患者自理能力、疼痛情况、并发症发生率及康复时间。结果:干预组日常生活功能评定表(BI)指数得分明显高于对照组(P<0.05);干预组术后12 h,24 h,48 h,72 h视觉模拟评分法(VAS)得分及并发症发生率均明显低于对照组(P<0.05);干预组进食时间、排便时间、下床活动时间及住院时间均明显短于对照组(P<0.05)。结论:加速康复理念下的术后活动量化标准可有效提高胃肠外科手术患者的自理能力,减轻疼痛,降低并发症发生率,缩短康复时间。

Objective:To observe the application effect of quantification standard of postoperative activities of patients with gastrointestinal surgery under the concept of accelerated rehabilitation.Methods:A total of 100 patients with gastrointestinal malignant tumors treated in hospital from January 2018 to June 2019 were grouped according to different wards.50 patients in ward A were the control group,and 50 patients in ward B For the intervention group.The control group adopted the traditional activity method,and the intervention group performed activities according to the quantified standard for postoperative activity revised by expert consultation.The self-care ability,pain situation,incidence of complications and recovery time of the two groups were compared.Results:The Barthel index score of the in-tervention group was significantly higher than that of the control group(P<0.05),the visual analogue scale(VAS)score and the incidence of complications of the intervention group at 12 h,24 h,48 h and 72 h were significantly lower than that of the control group(P<0.05).The time,defecation time,time to get out of bed and hospitalization time were significantly shorter than the control group(P<0.05).Conclusion:The quantitative standard of postoperative activities under the concept of accelerated rehabilitation can effectively improve the self-care ability of patients undergoing gastrointestinal surgery,and reduce pain and complications,and shorten the recovery time.
