复方雪莲烧伤膏对深Ⅱ度烫伤创面愈合的作用及机制

Effect of Compound Xuelian Burn Cream on DeepⅡDegree Scald Wound Healing and Its Mechanism of Action

目的探讨复方雪莲烧伤膏对深Ⅱ度烫伤大鼠模型创面愈合的作用和机制,为临床应用其治疗深Ⅱ度烫伤提供可靠的数据支持。方法将70只成年健康的SD大鼠随机分为A组(假烫伤组)10只、B组(单纯烫伤组)20只、C组(烧伤湿润膏对照组)20只和D组(复方雪莲烧伤膏组)20只,制备深Ⅱ度烫伤模型,A组除常规喂养外不做任何处理,B组给予创面外敷凡士林,C组给予创面外敷烧伤湿润膏,D组给予创面外敷复方雪莲烧伤膏。观察各组创面愈合时间和创面愈合率,于造模后第4、7、10、14天取材,检测各组创面组织含水量、CD68巨噬细胞含量、血管内皮生长因子(VEGF)、VEGFmRNA相对表达量和bcl-2、Bax蛋白表达水平。结果D组创面愈合时间短于B、C组,创面愈合率高于B、C组,创面组织含水量少于B、C组(均P<0.05);D组创面组织中VEGF含量、VEGFmRNA的相对表达量和bcl-2蛋白表达水平均高于A、B、C组,Bax蛋白表达水平低于A、B、C组(均P<0.05);D组的CD68巨噬细胞含量低于A组,高于B、C组(P<0.05),说明D组的CD68巨噬细胞含量调控在一定范围内。结论复方雪莲烧伤膏对深Ⅱ度烫伤创面有收敛作用,其通过调控巨噬细胞含量,促进VEGF分泌、VEGFmRNA的表达,上调bcl-2的同时下调Bax蛋白,从而促进创面愈合。

Objective To explore the effect of compound Xuelian burn ointment on wound healing of deepⅡdegree scald and its mechanism of action in rats,and to provide reliable data support for its clinical application in the treatment of deepⅡdegree scald.Methods Seventy healthy adult SD rats were randomly divided into four groups,and the model of deepⅡdegree scald was established.The sham-burn group(group A)received routine feeding without treatment.The simple burn group(group B)was given external application of Vaseline.The group C received external application of burn cream.The group D was treated with external application of compound Xuelian burn ointment.Wound healing time and healing rate were observed,and water and CD68 macrophage content in wound tissue were determined at 4,7,10 and 14 days after model establishment.In addition,the expression levels of vascular endothelial growth factor(VEGF),VEGF mRNA,Bcl-2 and Bax were measured in all the four groups.Results Compared with group B or C,wound healing time was shortened,wound healing rate was increased and water content of wound tissue was reduced in group D(P<0.05).The levels of VEGF,VEGF mRNA and Bcl-2 protein in group D were higher than those in group A,B or C,and the expression of Bax in group D was lower than that in group A,B or C(P<0.05).The content of CD68 macrophages in group D was lower than that in group A,but higher than that in group B or C(P<0.05),suggesting that CD68 macrophage content in group D was regulated within a certain range.Conclusion Compound Xuelian burn ointment has an astringent effect on deepⅡdegree scald.It can promote wound healing through regulating macrophage content,stimulating VEGF secretion,increasing VEGF mRNA expression,up-regulating Bcl-2 expression,and down-regulating Bax expression.
