胺碘酮治疗阵发性房颤的效果评价

Effect of amiodarone in the treatment of paroxysmal atrial fibrillation

目的:评价胺碘酮对阵发性房颤患者心脏电生理的影响。方法:回顾性选取阵发性房颤患者100例,依据治疗方法分为常规治疗组(n=50)和联合治疗组(n=50),两组均给予常规抗心律失常治疗,联合治疗组在此基础上给予胺碘酮治疗。比较两组的心脏电生理参数、复律占比、临床疗效、房颤转复时间、复律后心率、QT间期、不良反应发生情况。结果:联合治疗组患者的心房有效不应期升高幅度高于常规治疗组(P<0.05);复律占比<3 h、3~8 h均显著高于常规治疗组,>8 h占比显著低于常规治疗组(P<0.05);治疗总有效率高于常规治疗组(84.0%vs 64.0%,P<0.05);房颤转复时间显著长于常规治疗组(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论:胺碘酮能影响阵发性房颤患者心脏电生理而发挥抗心律失常作用。

Objective:To evaluate the effect of amiodarone on cardiac electrophysiology in patients with paroxysmal atrial fibrillation.Methods:One hundred patients with paroxysmal atrial fibrillation(PAF)were divided into a conventional treatment group(n=50)and a combined treatment group(n=50).The cardiac electrophysiological parameters,cardioversion ratio,clinical efficacy,conversion time of atrial fibrillation,heart rate after cardioversion,QT interval and the incidence of adverse reactions were compared between the two groups.Results:The increase of atrial effective refractory period in the combined treatment group was higher than that in the conventional treatment group(P<0.05).The ratio of cardioversion(<3 h)and(3-8 h)were significantly higher and the ratio of cardioversion(>8 h)was significantly lower in the combined treatment group than the conventional treatment group(P<0.05).The overall response rate of treatment was higher(84.0%vs 64.0%,P<0.05),the conversion time of atrial fibrillation was significantly longer(P<0.05)in the combined treatment group than the conventional treatment group.There were no significant differences in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Amiodarone can play an antiarrhythmic role by affecting cardiac electrophysiology in patients with paroxysmal atrial fibrillation.
