经脐单孔腹腔镜卵巢囊肿剥除术的应用价值及对患者卵巢功能指标的影响

Application value of transumbilical single-port laparoscopic ovarian cystectomy and its influence on ovarian function indexes of patients

目的探究经脐单孔腹腔镜卵巢囊肿剥除术的应用价值及对患者卵巢功能指标的影响。方法选取2018年10月至2020年10月我院收治的76例卵巢囊肿患者作为研究对象,按照随机数字表法将其分为观察组和对照组,每组38例。观察组给予经脐单孔腹腔镜卵巢囊肿剥除术,对照组给予传统三孔腹腔镜手术。比较两组患者的手术时间、术中出血量、术后排气时间、住院时间及疼痛程度,术前、术后1个月的卵巢功能指标及并发症发生情况。结果两组的手术时间比较,差异无统计学意义(P>0.05);观察组的术中出血量少于对照组,术后排气时间、住院时间短于对照组,VAS评分低于对照组,差异具有统计学意义(P<0.05)。术前、术后1个月,两组的E_(2)、FSH、LH水平比较,差异无统计学意义(P>0.05);术后1个月,两组的E_(2)水平均低于术前,FSH及LH水平均高于术前,差异具有统计学意义(P<0.05)。观察组的并发症总发生率为5.26%,明显低于对照组的21.05%,差异具有统计学意义(P<0.05)。结论经脐单孔腹腔镜卵巢囊肿剥除术具有创伤小、术后恢复快的优势,且能改善患者卵巢功能指标,并发症少,值得推广。

Objective To explore the application value of transumbilical single-port laparoscopic ovarian cystectomy and its influence on ovarian function indexes of patients.Methods A total of 76 patients with ovarian cyst treated in our hospital from October 2018 to October 2020 were selected as the research objects.The patients were divided into observation group and control group according to random number table method,with 38 cases in each group.The observation group was given transumbilical single-port laparoscopic ovarian cystectomy,and the control group was given traditional three-port laparoscopic surgery.The operation time,amount of intraoperative bleeding,postoperative exhaust time,hospital stay and degree of pain,ovarian function indexes before and 1 month after operation and occurrence of complications were compared between the two groups.Results There was no significant difference in operation time between the two groups(P>0.05);the amount of intraoperative bleeding in the observation group was less than that in the control group,the postoperative exhaust time and hospital stay were shorter than those in the control group,the VAS score was lower than that in the control group,and the differences were statistically significant(P<0.05).Before and 1 month after operation,there were no significant differences in the levels of E_(2),FSH and LH between the two groups(P>0.05);one month after operation,the level of E_(2) in the two groups was lower than that before operation,and the levels of FSH and LH were higher than those before operation,and the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was 5.26%,which was significantly lower than 21.05%in the control group,and the difference was statistically significant(P<0.05).Conclusion Transumbilical single-port laparoscopic ovarian cystectomy has the advantages of less trauma,rapid postoperative recovery,it can improved ovarian function indexes of patients,with fewer complications,which is worthy of promotion.
