葛根芩连汤联合利巴韦林治疗婴幼儿轮状病毒肠炎伴良性惊厥的临床效果

Clinical effect of Gegen Qinlian decoction combined with ribavirin in the treatment of infantile rotavirus enteritis complicated with benign convulsion

目的观察葛根芩连汤联合利巴韦林治疗婴幼儿轮状病毒肠炎伴良性惊厥的临床效果,为临床治疗提供依据。方法选取2019年1月至2021年1月我院接收的100例轮状病毒肠炎伴良性惊厥患儿为研究对象,根据治疗方法的不同将其分为对照组和观察组,每组50例。对照组给予利巴韦林治疗,观察组给予葛根芩连汤联合利巴韦林治疗。比较两组的预后相关指标、免疫功能、血清炎性因子指标、一氧化氮(NO)、硫化氢(H2S)、血清神经元特异性烯醇化酶(NSE)水平及临床疗效。结果观察组的住院时间、大便成形时间、退热时间、惊厥消失时间均短于对照组,治疗5 d后病毒转阴率高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)均升高,CD8^(+)均降低,且观察组的CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)高于对照组,CD8^(+)低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的IL-6、IL-8和TNF-α水平均低于治疗前,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的NO、NSE水平低于治疗前,H2S水平高于治疗前,且观察组的NO、NSE水平低于对照组,H2S水平高于对照组(P<0.05)。观察组的治疗总有效率为94.00%,显著高于对照组的80.00%,差异具有统计学意义(P<0.05)。结论葛根芩连汤联合利巴韦林治疗婴幼儿轮状病毒肠炎伴良性惊厥的效果显著,能有效缩短大便成形时间、退热时间及惊厥消失时间,提高病毒转阴率,改善T淋巴细胞亚群水平,降低炎性因子水平,改善患儿的预后效果。

Objective To observe the clinical effect of Gegen Qinlian decoction combined with ribavirin in the treatment of infantile rotavirus enteritis complicated with benign convulsion,and to provide basis for clinical treatment.Methods A total of 100 children with rotavirus enteritis complicated with benign convulsion received in our hospital from January 2019 to January 2021 were selected as the research objects,and the children were divided into control group and observation group according to the different treatment methods,with 50 cases in each group.The control group was treated with ribavirin,and the observation group was treated with Gegen Qinlian decoction combined with ribavirin.The prognosis related indexes,immune function,serum inflammatory factor indexes,nitric oxide(NO),hydrogen sulfide(H2S),serum neuron specific enolase(NSE)levels and clinical efficacy were compared between the two groups.Results The hospital stay,stool forming time,fever relieving time and convulsion disappearance time in the observation group were shorter than those in the control group,the negative conversion rate of virus after 5 d of treatment was higher than that in the control group,and the differences were statistically significant(P<0.05).After treatment,CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the two groups increased and CD8^(+)decreased,CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,and CD8^(+)was lower than that in the control group,the differences were statistically significant(P<0.05).After treatment,the levels of IL-6,IL-8 and TNF-αin the two groups were lower than those before treatment,and those in the observation group were lower than the control group,the differences were statistically significant(P<0.05).After treatment,the levels of NO and NSE in the two groups were lower than those before treatment,the level of H2S was higher than that before treatment,the levels of NO and NSE in the observation group were lower than those in the control group,and the level of H2S was higher than that in the control group(P<0.05).The total effective rate of treatment in the observation group was 94.00%,which was significantly higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion Gegen Qinlian decoction combined with ribavirin has a significant effect in the treatment of infantile rotavirus enteritis complicated with benign convulsion,which can effectively shorten the stool formation time,fever relieving time and convulsion disappearance time,improve the negative conversion rate of virus and the levels of T lymphocyte subsets,reduce the inflammatory factor level,and ameliorate the prognosis of children.
