上颌前牙区不翻瓣即刻牙种植的临床效果分析

Effect of Immediate Implant without Flap Turnover on Sesthetic Effect and Postoperative Pain Degree of Implant Patients in Maxillary Anterior Region

目的:探究上颌前牙区不翻瓣即刻牙种植的临床效果。方法:选择2015年6月-2019年6月就诊的上颌前牙区需修复的80例患者,根据就诊顺序分为对照组(40例)和试验组(40例)。试验组接受不翻瓣即刻牙种植术修复,对照组接受翻瓣即刻牙种植术修复,两组均术后6个月随访。观察两组手术相关指标,术后6个月时牙槽软硬组织情况,术前、术后6个月的美学效果、疼痛程度及术后并发症发生情况。结果:试验组手术时间、术中出血量、术后消肿时间及术后抗菌药物应用时间均短于对照组,差异有统计学意义(P<0.05)。术后6个月,试验组近中牙龈附着退缩、远中牙龈附着退缩、牙槽骨唇颊侧近中骨板吸收、牙槽骨唇颊侧远中骨板吸收指标均低于对照组,差异有统计学意义(P<0.05)。术后6个月,两组VAS评分降低,且试验组低于对照组,PES评分升高,且试验组高于对照组,差异有统计学意义(P<0.05)。术后6个月,试验组并发症发生率为25.00%,低于对照组的52.50%,差异有统计学意义(P<0.05)。结论:相较于翻瓣即刻牙种植术,不翻瓣即刻牙种植术可改善上颌前牙区种植患者的手术相关指标,提高美学效果,减轻术后疼痛程度,安全性高,值得临床推广应用。

Objective To explore the clinical effect of immediate dental implantation without flap turnover in maxillary anterior region.Methods 80 patients with maxillary anterior teeth requiring restoration who visited our hospital from June 2015 to June 2019 were selected and divided into the control group (40 cases) and the test group (40 cases) according to the admission order.The test group received immediate dental implant repair without flap turnover,while the control group received immediate dental implant repair with flap turnover.The two groups were followed up for 6 months after operation.The operation related indexes,alveolar soft and hard tissues at 6 months after operation,aesthetic effect,pain degree and postoperative complications at 6 months before and after operation were observed.Results The operation time,blood loss during operation,detumescence time and antibacterial drug application time in the test group were shorter than those in the control group (P<0.05).6 months after operation,the indexes of proximal gingival recession,distal gingival recession,alveolar bone labial buccal proximal bone plate absorption and alveolar bone labial buccal distal bone plate absorption in the test group were lower than those in the control group (P<0.05).6 months after the operation,the VAS score of the two groups decreased,and the test group was lower than the control group,the PES score increased,and the test group was higher than the control group (P<0.05).6 months after operation,the incidence of complications in the test group was 25.00%,lower than 52.50% of the control group(P<0.05).Conclusion Compared with immediate dental implantation without flap turnover,immediate dental implantation without flap turnover can improve the operation-related indexes of implant patients in maxillary anterior region,improve aesthetic effect,reduce postoperative pain,and has high safety,which is worthy of promotion.
