艾儒棣教授从“郁热相火”论治肝郁血热型玫瑰痤疮 被引量:4

Professor Ai Rudi’s Treatment of Rose Acne with Liver Depression and Blood Heat from Perspective of“Stagnation of Heat and Ministerial Fire”

摘要 玫瑰痤疮是一种以面部阵发性潮红、持久性红斑、丘疹并伴瘙痒、疼痛不适为主临床表现的皮肤病。中医外科名家艾儒棣教授提出郁热相火理论,认为肝郁血热型玫瑰痤疮发病关键在于君火不安,相火妄动,郁热内生,肝肾阴亏;相火挟湿热、瘀热上攻头面为病。创新性提出了“安君火、泻相火、解郁热”的原则,治疗上应着眼于肝,顾及心肾,滋养阴液、调理气机。本文通过介绍艾儒棣教授从郁热相火治疗肝郁血热型玫瑰痤疮临床经验及典型医案,补充了对肝郁血热型玫瑰痤疮病机与治法的认识思路,进而体会“相火论”在玫瑰痤疮诊治中应用的临床思维。 Rose acne is a skin disease characterized by paroxysmal flushing,persistent erythema,papules with itching,pain and discomfort.Professor Ai Rudi,a famous surgeon of traditional Chinese medicine,put forward the theory of stagnation of Heat andministerial Fire,and thought that the key to the onset of rose acne of Liver depression and Blood Heat type lies in the restlessness of monarch Fire,the impulsion of ministerial Fire,endogenous stagnation of Heat and Yin deficiency of Liver and Kidney;ministerial Fire with Damp Heat and Blood stasis Heat attacking the head and face.Innovatively put forward the principle of“calming the monarch Fire,purging the ministerial Fire,and relieving depression and Heat”,which should focus on the Liver,take care of the Heart and Kidney,nourish Yin fluid and regulate Qi.This paper introduces Professor Ai Rudi’s clinical experience and typical medical cases in treating rose acne with Liver depression and Blood-Heat from stagnation of Heat and Fire,so as to supplement his understanding of the pathogenesis and treatment of rose acne withLiver depression and Blood-Heat,and then realize the clinical thinking of the application of“ministerial Fire theory”in the diagnosis and treatment of rose acne.
