摘要
目的分析HIV-1感染者在长期抗反转录病毒治疗(antiretroviral therapy,ART)前后血浆中p24抗体和总HIV-1特异性抗体水平变化特点及其与HIV-1储存库关系。方法选取2009年11月—2013年7月于我中心接受ART满5年的27例HIV-1感染者作为研究对象,采用荧光素酶免疫吸附试验和酶联免疫吸附试验,分别检测患者接受ART 0、1、3、5年时间点的p24抗体和总HIV-1特异性抗体的水平,同时对其与总HIV-1 DNA和细胞相关RNA(cell-associated RNA,CA-RNA)的水平进行相关性分析。结果在ART 0、1、3、5年时,p24抗体水平分别为2.450(1.910~5.460)、1.460(1.080~2.160)、1.280(1.120~1.800)、1.570(1.090~2.180)LU;总HIV-1特异性抗体水平分别为3.378(3.157~3.710)、3.489(3.237~3.622)、3.446(2.742~3.573)、3.336(2.860~3.566)。p24抗体与总HIV-1 DNA和CA-RNA的水平均呈正相关(r分别为0.407、0.305,P均<0.05);总HIV特异性抗体水平与总HIV-1 DNA和CA-RNA水平没有相关性(r分别为0.155、0.165,P均>0.05)。结论总HIV-1特异性抗体水平在ART期间没有明显变化,且与HIV-1储存库大小没有相关性,p24抗体水平随着ART时间增加呈逐渐降低的趋势,与HIV-1储存库的大小呈正相关,可为HIV-1特异性抗体和HIV-1储存库大小关系的研究提供依据。
Objective To analyze the changes in p24 antibody levels and total HIV-1 specific antibody levels and their relationship with HIV-1 reservoirs in patients with HIV-1 infected before and after long-term antiretroviral therapy(ART).Methods Twenty-seven HIV-1 infected patients who had been on ART for 5 years in our center from November 2009 to July 2013 were included in this study. The levels of p24 antibodies and total HIV-1 specific antibodies at the time points of 0, 1, 3 and 5 years after ART were measured by luciferase immunosorbent assay and enzyme-linked immunosorbent assay, respectively,and correlated with the levels of total HIV-1 DNA and cell-associated RNA(CA-RNA). Results At 0, 1, 3, and 5 years of ART,the levels of p24 antibody were 2.450(1.910-5.460), 1.460(1.080-2.160), 1.280(1.120-1.800), and 1.570(1.090-2.180) LU,respectively;the levels of total HIV-1 specific antibody were 3.378(3.157-3.710), 3.489(3.237-3.622), 3.446(2.742-3.573) and 3.336(2.860-3.566), respectively. p24 antibody was positively correlated with both total HIV-1 DNA and CA-RNA levels(r=0.407, 0.305, P < 0.05). While the levels of total HIV-1 specific antibody was no correlated with both total HIV-1 DNA and CARNA levels(r=0.155, 0.165, P >0.05). Conclusions The levels of total HIV-1 specific antibody did not change significantly during ART, and did not correlate with the size of HIV-1 reservoir. The levels of p24 antibody decreased gradually with increasing ART duration, and was positively correlated with HIV-1 reservoir size. The study might provide new insight into the relationship between HIV-1 specific antibodies and HIV-1 reservoir size.
