经尿道钬激光膀胱肿瘤切除术治疗老年非肌层浸润性膀胱癌患者的效果

Effects of transurethral holmium laser resection of bladder tumors for elderly patients with non-muscle invasive bladder cancer

目的:观察经尿道钬激光膀胱肿瘤切除术(THLRBT)治疗老年非肌层浸润性膀胱癌(NMIBC)患者的效果。方法:选取100例老年NMIBC患者为研究对象,按照随机数字表法分为对照组和观察组各50例。对照组行经尿道膀胱肿瘤切除术(TURBT)治疗,观察组行THLRBT治疗。比较两组临床相关指标(手术时间、术后下床时间、导尿管留置时间、住院时间)水平、排尿功能指标(最大逼尿肌压力、最大尿流率)水平、并发症发生率和复发率。结果:观察组导尿管留置时间和住院时间均短于对照组,差异有统计学意义(P<0.05);两组手术时间和术后下床时间比较,差异无统计学意义(P>0.05);术后,两组最大逼尿肌压力和最大尿流率均高于术前,且观察组高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为2.00%、复发率为2.00%,分别低于对照组的16.00%、18.00%,差异均有统计学意义(P<0.05)。结论:THLRBT治疗老年NMIBC患者可缩短术后导尿管留置时间和住院时间,提高排尿功能指标水平,以及降低并发症发生率和复发率,优于TURBT治疗效果。

Objective:To observe effects of transurethral holmium laser resection of bladder tumors(THLRBT)for elderly patients with non-muscle invasive bladder cancer(NMIBC).Methods:100 elderly patients with NMIBC were selected as the research objects,and were divided into control group and observation group according to the random number table method,50 cases in each group.The control group was treated with TURBT,while the observation group was treated with THLRBT.The levels of clinically relevant indicators(operation time,postoperative ambulation time,catheter indwelling time,hospitalization time),the urination function indicator levels(maximum detrusor pressure,maximum urine flow rate),the complication rate,and the recurrence were compared between the two groups.Results:The catheter indwelling time and the hospitalization time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).There were no significant difference in the operation time and the postoperative ambulation time between the two groups(P>0.05).After the surgery,the maximum detrusor pressure and the maximum urine flow rate in both groups were higher than those before the surgery;those of the observation group were higher than those of the control group;the differences were statistically significant(P<0.05).Further,the complication rate and the recurrence rate of the observation group were 2.00%and 2.00%,which were lower than 16.00%and 18.00%of the control group,respectively,and the differences were statistically significant(P<0.05).Conclusions:THLRBT for the elderly NMIBC patients can shorten the postoperative catheter indwelling time and the hospitalization time,improve the levels of urination function indicators,and reduce the complication rate and the recurrence rate.Moreover,it is superior to TURBT treatment.
