摘要
目的:观察乌司他丁联合胰岛素泵治疗糖尿病酮症酸中毒(DKA)患者的效果。方法:选取74例DKA患者为研究对象,按照随机数字表法分为观察组与对照组各37例。对照组应用胰岛素泵治疗,观察组在对照组基础上联合注射用乌司他丁治疗,比较两组临床疗效、β-羟丁酸(β-HB)水平、乳酸(LAC)水平和炎性相关指标[肿瘤坏死因子-α(TNF-α)、高迁移率族蛋白B1(HMGB1)、C反应蛋白(CRP)及脂蛋白相关磷脂酶A2(Lp-PLA2)]水平。结果:观察组治疗总有效率为91.89%(34/37),明显高于对照组的72.97%(27/37),差异有统计学意义(P<0.05);治疗后,两组β-HB和LAC水平均低于治疗前,且观察组明显低于对照组,差异有统计学意义(P<0.05);治疗后,两组TNF-α、HMGB1、CRP和Lp-PLA2水平均低于治疗前,且观察组明显低于对照组,差异有统计学意义(P<0.05)。结论:乌司他丁联合胰岛素泵治疗DKA患者可提高治疗总有效率,降低β-HB、LAC及炎性相关指标水平,效果优于单纯胰岛素泵治疗。
Objective:To observe effects of Ulinastatin combined with insulin pump in treatment of patients with diabetic ketoacidosis(DKA).Methods:74 patients with DKA were selected as the research objects,and were divided into observation group and control group according to the random number table method,37 cases in each group.The control group was treated with insulin pump,while the observation group was treated with Ulinastatin for injection on the basis of that of the control group.The clinical efficacy,theβ-hydroxybutyric acid(β-HB)level,the lactic acid(LAC)level and the inflammatory related indexes[tumor necrosis factor-α(TNF-α),and high mobility group box protein B1(HMGB1),C-reactive protein(CRP)and lipoprotein-associated phospholipase A2(Lp-PLA2)]levels were compared between the two groups.Results:The total effective rate of the observation group was 91.89%(34/37),which was significantly higher than 72.97%(27/37)of the control group,the difference was statistically significant(P<0.05).After the treatment,the levels ofβ-HB and LAC in the two groups were lower than those before the treatment;those of the observation group were significantly lower than those of the control group;and the differences were statistically significant(P<0.05).After the treatment,the levels of TNF-α,HMGB1,CRP and Lp-PLA2 in the two groups were lower than those before the treatment;those of the observation group were significantly lower than those of the control group;and the differences were statistically significant(P<0.05).Conclusions:Ulinastatin combined with insulin pump in the treatment of the DKA patients can improve the total effective rate of treatment and reduce the levels ofβ-HB,LAC and inflammatory related indexes.Moreover,it is superior to single insulin pump treatment.
