醒脑开窍针剌疗法联合康复训练治疗卒中后偏瘫患者的效果

Effects of Xingnao Kaiqiao acupuncture combined with rehabilitation training in treatment of post-stroke hemiplegia patients

目的:观察醒脑开窍针剌疗法联合康复训练治疗卒中后偏瘫患者的效果。方法:选取76例卒中后偏瘫患者为研究对象,按随机数字表法将其分为对照组与观察组各38例。对照组进行康复训练,观察组在对照组基础上联合醒脑开窍针剌疗法治疗,比较两组治疗前、治疗3个月后神经功能缺损程度[美国国立卫生研究院卒中量表(NIHSS)]评分、肢体运动功能[简化Fugl-Meyer运动功能量表(FMA)]评分、中医证候积分和脑动脉[大脑前动脉(ACA)、大脑中动脉(MCA)、大脑后动脉(PCA)]平均血流流速。结果:治疗后,观察组NIHSS评分低于对照组,差异有统计学意义(P<0.05);观察组上、下肢FMA评分均高于对照组,差异有统计学意义(P<0.05);观察组头晕头痛、口舌歪斜、肢体麻木等中医证候积分均低于对照组,差异有统计学意义(P<0.05);观察组ACA、MCA、PCA等脑动脉平均血流流速均高于对照组,差异有统计学意义(P<0.05)。结论:醒脑开窍针剌疗法联合康复训练治疗卒中后偏瘫患者可提高脑动脉平均血流流速和肢体运动功能评分,降低中医证候积分和NIHSS评分,其效果优于单纯康复训练治疗。

Objective:To observe effects of Xingnao Kaiqiao acupuncture combined with rehabilitation training in treatment of post-stroke hemiplegia patients.Methods:76 patients with post-stroke hemiplegia were selected as the research objects,and were divided into control group and observation group according to the random number table method,38 cases in each group.The control group was given rehabilitation training,while the observation group was treated with Xingnao Kaiqiao acupuncture therapy on the basis of that of the control group.The neurological deficit degree[National Institutes of Health Stroke Scale(NIHSS)]score,the limb motor function[simplified Fugl-Meyer motor function scale(FMA)]score,the TCM syndrome score and the mean cerebral artery blood flow velocity[anterior cerebral artery(ACA),middle cerebral artery(MCA),posterior cerebral artery(PCA)]levels were compared between the two groups before treatment and 3 months after the treatment.Results:After the treatment,the NIHSS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The FMA scores of upper and lower limbs in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The scores of TCM syndromes such as dizziness,headache,skewed tongue and limb numbness in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the average blood flow velocity levels of ACA,MCA and PCA in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:Xingnao Kaiqiao acupuncture therapy combined with rehabilitation training in the treatment of post-stroke hemiplegia can improve the average blood flow velocities of cerebral arteries and limb motor function scores,and reduce TCM syndrome scores and the NIHSS scores.Moreover,it is superior to single rehabilitation training.
