彩色多普勒超声检查在心肌重构高血压患者左室舒张功能评估中的应用价值

Application value of color Doppler ultrasonography in evaluation of left ventricular diastolic function in hypertensive patients with myocardial remodeling

目的:探讨彩色多普勒超声检查在心肌重构高血压患者左室舒张功能评估中的应用价值。方法:选取90例原发性高血压患者为研究对象,根据不同左室质量指数(LVMI)及相对室壁厚度分组,其中左室心肌正常构型35例(对照组),左室心肌重构55例(观察组),比较两组一般资料及临床相关指标[性别、年龄、收缩压(SBP)、舒张压(DBP)、心率(HR)、体质量指数(BMI)、左室射血分数(LVEF)、左心房面积、左室质量]水平、左室舒张功能相关指标[舒张早期二尖瓣血流速度E峰(E)、舒张早期二尖瓣血流速度A峰(A)、E/A、二尖瓣瓣环峰值运动速度(E’)、舒张晚期峰值运动速度(A’)、E’/A’、E/E’]水平、分析左心房面积与左室舒张功能参数E/E’、E/A值的相关性。结果:两组性别、年龄、HR、DBP、BMI、LVEF比较,差异均无统计学意义(P>0.05);观察组左室质量、SBP明显高于对照组,左心房面积大于对照组,差异均有统计学意义(P<0.05);两组E、A、A’、E/E’比较,差异均无统计学意义(P>0.05);观察组E/A、E’、E’/A’明显低于对照组,差异均有统计学意义(P<0.05);高血压患者的左心房面积与E/E’值存在相关性(r=0.696,P<0.05),与E/A值无相关性(r=0.170,P>0.05)。结论:彩色多普勒超声检查在心肌重构高血压患者左室舒张功能评估中具有较高的应用价值。

Objective:To explore application value of color Doppler ultrasonography in evaluation of left ventricular diastolic function in hypertensive patients with myocardial remodeling.Methods:90 patients with essential hypertension were selected as the research subjects.According to different levels of left ventricular mass index(LVMI)and relative ventricular wall thickness,they were divided into normal left ventricular myocardial configuration group(control group,35 cases)and o left ventricular myocardial remodeling group(observation group,55 cases).The general data,the clinical related indicator levels[sex,age,systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR),body mass index(BMI),left ventricular ejection fraction(LVEF),left atrial area,left ventricular mass],the left ventricular diastolic function-related indicator levels[the mitral valve diastolic blood flow spectrum E peak(E),mitral valve diastolic blood flow spectrum A peak(A),E/A,mitral annulus early diastolic peak velocity(E’),and late diastolic peak velocity(A’),E’/A’,E/E’]were compared between the two groups.Further,the correlations among the left atrial area and the left ventricular diastolic function parameters E/E’and E/A were analyzed.Results:There were no significant differences in gender,age,HR,DBP,BMI and LVEF between the two groups(P>0.05).The left ventricular mass and SBP in the observation group were significantly higher than those in the control group,and the left atrial area was larger than that in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in the levels of E,A,A’,and E/E’between the two groups(P>0.05).The levels of E/A,E’,and E’/A’in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The left atrial area was correlated with the E/E’value in hypertensive patients(r=0.696,P<0.05),but not correlated with the E/A value(r=0.170,P>0.05).Conclusions:Color Doppler ultrasonography has a high application value in the evaluation of left ventricular diastolic function in hypertensive patients with myocardial remodeling.
