血清半乳糖凝集素-3、D-二聚体、内皮素-1水平与慢性肺心病病情程度的相关性

Correlations among serum galectin-3,D-dimer and endothelin-1 levels and severity of chronic pulmonary heart disease

目的:观察血清半乳糖凝集素-3(Galectin-3)、D-二聚体(D-D)、内皮素-1(ET-1)水平与慢性肺源性心脏病(CPHD)病情程度的相关性。方法:选取67例CPHD患者为研究对象,其中代偿期患者36例,设为代偿组;失代偿期患者31例,设为失代偿组。另选取同期30名健康体检者为对照组。检测三组血清Galectin-3、D-D、ET-1、N端脑钠肽前体(NT-proBNP)、血气指标[动脉血氧分压(PO_(2))、动脉血二氧化碳分压(PCO_(2))]水平、肺动脉压指标[肺动脉收缩压(PASP)、肺动脉舒张压(PADP)]水平,比较三组Galectin-3、D-D、ET-1、NT-proBNP、PO_(2)、PCO_(2)、PASP和PADP水平,采用Pearson相关性分析,分析Galectin-3、D-D、ET-1水平与NT-proBNP、PO_(2)、PCO_(2)、PASP、PADP水平的相关性。结果:失代偿组血清Galectin-3、D-D、ET-1、NT-proBNP水平均高于代偿组和对照组,且代偿组高于对照组,差异有统计学意义(P<0.05)。失代偿组PCO_(2)、PASP、PADP水平均高于代偿组和对照组,且代偿组高于对照组;失代偿组PO_(2)水平低于代偿组和对照组,且代偿组低于对照组,差异均有统计学意义(P<0.05)。Pearson相关性分析结果显示,血清Galectin-3水平与NT-proBNP、PCO_(2)、PASP、PADP水平均呈正相关(r>0,P<0.05),与PO_(2)水平呈负相关(r<0,P<0.05);血清D-D水平与NT-proBNP、PCO_(2)、PASP、PADP水平均呈正相关(r>0,P<0.05),与PO_(2)水平呈负相关(r<0,P<0.05);血清ET-1水平与NT-proBNP、PCO_(2)、PASP、PADP水平均呈正相关(r>0,P<0.05),与PO_(2)水平呈负相关(r<0,P<0.05)。结论:血清Galectin-3、D-D、ET-1水平与CPHD病情严重程度显著相关。

Objective:To observe correlations among levels of serum Galectin-3(Galectin-3),D-dimer(D-D),and endothelin-1(ET-1)and severity of chronic pulmonary heart disease(CPHD).Methods:67 CPHD patients were selected as the research objects.Among them,36 patients in the compensatory phase were set as the compensation group,while 31 patients in the decompensated period were set as the decompensated group.In addition,30 healthy subjects who underwent physical examination in our hospital during the same period were selected as control group.The serum levels of Galectin-3,D-D,ET-1,N-terminal pro-brain natriuretic peptide(NT-proBNP),the blood gas index levels[arterial partial pressure of oxygen(PO_(2)),arterial partial pressure of carbon dioxide(PCO_(2))],the pulmonary artery pressure index levels[pulmonary artery systolic pressure(PASP),pulmonary artery diastolic pressure(PADP)]of the three groups were detected.The levels of Galectin-3,D-D,ET-1,NT-proBNP,PO_(2),PCO_(2),PASP and PADP were compared among the three groups.Pearson correlation analysis was used to analyze the correlations among the levels of Galectin-3,D-D and ET-1 and the levels of NT-proBNP,PO_(2),PCO_(2),PASP and PADP.Results:The serum levels of Galectin-3,D-D,ET-1 and NT-proBNP in the decompensated group were higher than those in the compensation group and the control group;those of the compensation group were higher than those of the control group;and the differences were statistically significant(P<0.05).The levels of PCO_(2),PASP and PADP in the decompensated group were higher than those in the compensation group and the control group,and those of the compensation group were higher than those of the control group;the PO_(2) level in the decompensated group was lower than that in the compensation group and the control group,and that of the compensation group was lower than that of the control group;and the differences were statistically significant(P<0.05).The Pearson correlation analysis showed that the serum Galectin-3 level was positively correlated with the NT-proBNP,PCO_(2),PASP,and PADP levels(r>0,P<0.05),and negatively correlated with the PO_(2) level(r<0,P<0.05);the serum D-D level was positively correlated with the NT-proBNP,PCO_(2),PASP,PADP levels(r>0,P<0.05),and negatively correlated with the PO_(2) level(r<0,P<0.05);and the serum ET-1 level was positively correlated with the NT-proBNP,PCO_(2),PASP and PADP levels(r>0,P<0.05),and was negatively correlated with the PO_(2) level(r<0,P<0.05).Conclusions:The serum Galectin-3,D-D and ET-1 levels are significantly correlated with the severity of CPHD.
