补肾强脊汤配合针灸治疗强直性脊柱炎疗效观察及对关节活动度的影响

Curative Effect of Bushen Qiangji Tang Combined with Acupuncture and Moxibustion on Ankylosing Spondylitis and its Effect on Joint Range of Motion

目的:观察补肾强脊汤内服配合针灸对强直性脊柱炎患者关节活动度及临床治疗效果的影响。方法:选取64例强直性脊柱炎患者为研究对象,按随机数字表法分为对照组与观察组各32例。对照组仅采用针灸治疗,观察组在对照组基础上加用补肾强脊汤治疗,观察2组临床疗效、关节活动度及炎症因子水平。结果:治疗后,观察组临床总有效率93.75%,高于对照组71.88%(P<0.05)。与同组治疗前比较,治疗后2组脊柱活动度Schober试验及胸廓呼吸差均升高(P<0.05),颌-柄距、指-地距、枕-墙距及白细胞介素-6 (IL-6)、肿瘤坏死因子-α(TNF-α)、C-反应蛋白(CRP)水平均降低(P<0.05);与对照组治疗后比较,观察组治疗后Schober试验及胸廓呼吸差均较高(P<0.05),颌-柄距、指-地距、枕-墙距及IL-6、TNF-α、CRP水平均较低(P<0.05)。结论:补肾强脊汤内服配合针灸可显著提高强直性脊柱炎患者的临床疗效,改善患者关节活动度,降低炎症因子水平。

Objective:To observe the effect of the therapy of oral administration of Bushen Qiangji tang combined with acupuncture and moxibustion on joint range of motion and clinical effects in patients with ankylosing spondylitis. Methods:A total of 64 patients with ankylosing spondylitis were selected and divided into the control group and the observation group according to the random number table method,with 32 cases in each group. The control group was treated with acupuncture and moxibustion,and the observation group was additionally treated with Bushen Qiangji tang based on the treatment of the control group. The clinical effects,joint range of motion and levels of inflammatory factors in the two groups were observed.Results:After treatment,the total effective rate was 93.75% in the observation group,higher than that of 71.88% in the control group(P<0.05). After treatment,the Schober test of spine range of motion and differences of chest range of motion during breathing in the two groups were increased when compared with those before treatment(P<0.05),and the distance between jaw and sternal manubrium,distance between finger and ground as well as distance between occipital bone and wall,and the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α) and C-reactive protein(CRP) were decreased(P<0.05);after treatment,the Schober test and differences of chest range of motion during breathing in the observation group were higher than those in the control group(P<0.05), and the distance between jaw and sternal manubrium, distance between finger and ground as well as distance between occipital bone and wall,and the levels of IL-6,TNF-α and CRP were lower(P<0.05). Conclusion:The therapy of oral administration of Bushen Qiangji tang combined with acupuncture and moxibustion can significantly enhance the clinical effect of patients with ankylosing spondylitis,improve their joint range of motion and reduce the levels of inflammatory factors.
