两地汤二至丸合方颗粒联合针灸对虚热型经期延长的疗效观察

The clinical efficacy of combination granules of Liangdi decoction and Erzhi pill combined with acupuncture on hot type prolonged menstruation

目的观察口服两地汤二至丸合方颗粒配合针灸穴位疗法对于虚热型经期延长治疗的有效性。方法选择2018年5~10月妇科诊治的虚热型经期延长患者60例,随机为对照组和观察组。对照组患者治疗使用西药甲羟孕酮,观察组使用针灸配合两地汤二至丸合方颗粒治疗,共治疗3个月经周期。比较2组临床疗效、中医证候疗效、中医证候积分变化、激素水平、月经经期及黄体期天数、凝血相关指标、不良反应。结果观察组治疗有效率为96.67%明显高于对照组的73.33%(P<0.05);观察组中医证候有效率为96.67%明显高于对照组的70.00%(P<0.05);观察组治疗后经期延长、经色经质、经量、五心烦热、潮热颧红、口咽干燥、小便短赤等中医证候积分明显低于对照组(P<0.05);观察组患者FSH、LH、E2、P指标变化与对照组比较,差异无统计学意义(P>0.05),观察组的月经周期天数、黄体期天数明显短于对照组(P<0.05);观察组患者治疗后PT、APTT指标变化明显,且较对照组患者短,PLT指标高于对照组(P<0.05),2组FIB比较差异无统计学意义(P>0.05)。结论中药汤剂联合针灸治疗虚热型经期延长疗效显著,能改善凝血功能,减轻中医证候,缩短黄体期及月经经期,达到良好的调整月经量、色、质作用,且安全性高,无明显不良反应,具有积极的临床意义。

Objective To observe the clinical efficacy of combination granules of Liangdi decoction and Erzhi pill combined with acupuncture on hot type prolonged menstruation.Methods Sixty patients with hot type prolonged menstruation,who were treated in our hospital from May 2018 to October 2018 were randomly divided into control group and observation group.The patients in control group were treated by medroxyprogesterone,while those in observation group were treated by combination granules of Liangdi decoction and Erzhi pill combined with acupuncture,with a treatment course of three menstrual cycles.The TCM syndrome efficacy,TCM syndrome integral changes,hormone levels,menstrual and luteal days,coagulation-related indicators and adverse reactions were observed and compared between the two groups.Results The effective rate in observation group(96.67%)was significantly higher than that(73.33%)in control group(P<0.05).The effective rate of TCM syndrome in observation group was 96.67%,which was significantly higher than that(70.00%)in control group(P<0.05).Moreover,the TCM syndrome scores in were significantly lower than those in control group(P<0.05).The changes of FSH,LH,E2 and P in observation group were similar to those in control group(P>0.05).The menstrual days and luteal phase day in observation group were significantly shorter than those in control group(P<0.05).The changes of Pt and APTT indexes in observation group were significant,and the PLT levels in observation group were significantly lhigher than those in control group(P<0.05).In addition,there were no significant differences in FIB levels between the two groups(P>0.05).Conclusion TCM decoction combined with acupuncture is quite effective in treating hot type prolonged menstruation,which can improve blood coagulation function,reduce TCM syndromes,shorten luteal phase and menstrual period and regulate menstrual volume,color and quality,which has important clinical significance.
