心理干预联合系统化护理对妊娠合并宫颈机能不全宫颈环扎术患者不良情绪的影响

Effects of psychological intervention combined with systematic nursing on adverse emotions in puerperas undergoing cervical cerclage due to pregnancy combined with cervical incompetence

目的探讨心理干预联合系统化护理模式在改善宫颈环扎术产妇消极情绪及妊娠结局中的应用价值。方法选择2019年4月至2021年4月收治的由于宫颈机能不全而行宫颈环扎术的80例产妇,随机分为试验组和对照组,每组40例。对照组进行系统护理模式,试验组在系统护理的基础上联合心理护理干预。比较2组产妇在抑郁自评量表(SDS)、焦虑自评量表(SAS)及孕周、新生儿Apgar评分、新生儿体重的差异。结果护理后2组产妇SAS评分及SDS评分均明显降低,且试验组产妇评分降低更显著(P<0.05);试验组平均孕周,显著高于对照组(P<0.05),试验组新生儿的Apgar评分、体重显著高于对照组(P<0.05);试验组的早产率为72.5%低于对照组的82.5%(P<0.05)。结论在宫颈机能不全患者宫颈环扎术围术期护理中,密切观察病情、预防感染、适当补充营养及营造舒适环境等系统化护理的同时辅助心理护理干预,能有效改善患者的心理状态,对于患者的病情恢复有积极意义。

Objective To investigate the effects of psychological intervention combined with systematic nursing on adverse emotions in puerperas undergoing cervical cerclage due to pregnancy combined with cervical incompetence.Methods Eighty puerpera with cervical cerclage due to cervical incompetence,who were treated in our hospital from April 2019 to April 2021,were randomly divided into experimental group and control group,with 40 cases in each group.The puerperas in control group were treated by systematic nursing,however,those in experimental group,on the basis of control group,were treated by psychological nursing intervention.The differences of self rating depression scale(SDS),self rating anxiety scale(SAS),gestational age,neonatal Apgar score and neonatal weight were observed and compared between the two groups.Results After nursing,the SAS scores and SDS scores were significantly decreased in both group,and the decrease in experimental group were more significant(P<0.05).The mean gestational age of the puerperas,the scores of Apgar and neonatal weight in experimental group were significantly higher than those in control group(P<0.05).The premature delivery rate in experimental group was 72.5%,which was significantly lower than that(82.5%)in control group(P<0.05).Conclusion Systematic nursing combined with psychological intervention,in nursing puerperas undergoing cervical cerclage due to pregnancy combined with cervical incompetence,can effectively improve the psychological state of the puerperas,which is significant for their recovery.
