超声引导下CRB、SGB联合药物治疗颈项部PHN的临床效果及不良反应

Effects and Adverse Reactions of Ultrasound-guided CRB and SGB Combined with Drugs in the Treatment of Cervical PHN

目的:分析带状疱疹后遗神经痛(postherpetic neuralgia,PHN)在超声引导下颈神经根阻滞(Cervical nerve root block,CRB)、星状神经节阻滞(Stellate ganglion block,SGB)联合药物治疗的效果及不良反应。方法:选取某院2018年2月~2020年9月接收的74例PHN患者采用随机数表法分为对照组和研究组各37例,两组均进行常规药物治疗,对照组给予在超声引导下SGB治疗,研究组在对照组的治疗方案的基础上实施CRB治疗。观察两组疼痛程度(VRS)、应激激素水平、睡眠质量(SRSS)及不良反应情况。结果:对比两组治疗前VRS评分、应激激素水平、SRSS评分无差异(P>0.05);研究组治疗1周、1个月、3个月VRS评分、应激激素水平、SRSS评分、不良反应发生率低于对照组(P<0.05)。结论:超声引导下CRB、SGB联合药物治疗PHN能显著减轻患者疼痛情况,在保证用药安全性的同时,降低应激激素水平,改善睡眠质量。

Objective:To analyze the efficacy and adverse reactions of ultrasound-guided cervical nerve root block(CRB)and stellate ganglion block(SGB)combined with drug therapy for postherpetic neuralgia(PHN).Methods:74 cases of PHN patients admitted to a hospital from February 2018 to September 2020 were selected and divided into two groups by random number table method.Both groups received conventional drug treatment,then the control group(37 cases)received ultrasound-guided SGB treatment,and the study group(37 cases)received CRB treatment on the basis of the treatment plan of the control group.Pain degree(VRS),stress hormone level,sleep quality(SRSS)and adverse reactions were observed in the two groups.Results:There was no difference in VRS scores stress hormone level and SRSS score between the two groups before treatment(P>0.05).The VRS score,stress hormone level,SRSS score and the incidence of adverse reactions in the study group were lower than those in the control group at 1 week,1 month and 3 months after treatment(P<0.05).Conclusion:Ultrasound-guided CRB and SGB combined with drug therapy for PHN can significantly reduce pain,reduce stress hormone levels and improve sleep quality while ensuring drug safety.
