腔镜腋窝淋巴结清扫术结合保留乳房手术治疗早期乳腺癌的远期疗效

Long Term Efficacy of Endoscopic Axillary Lymph Node Dissection Combined with Breast Conserving Surgery in the Treatment of Early Breast Cancer

目的:探讨腔镜腋窝淋巴结清扫术结合保留乳房手术治疗早期乳腺癌的远期疗效。方法:回顾性分析55例早期乳腺癌患者的临床资料,其中25例患者接受乳腺单纯切除联合腋窝淋巴结清扫(对照组),30例患者接受腔镜腋窝淋巴结清扫术联合保留乳房手术(观察组),比较两组手术治疗情况、并发症与预后情况。结果:两组手术时间、淋巴结清扫个数、局部复发率、远处转移率与死亡率均无明显差异(P>0.05);相较于对照组,观察组具备更低术中出血量与并发症发生率,更短住院时间,更高乳房美观度优良率(P<0.05)。结论:腔镜腋窝淋巴结清扫术结合保留乳房手术治疗早期乳腺癌的远期疗效确切,可取得与乳腺切除相似的预后情况,且乳房美观度更好。

Objective:To evaluate the long-term efficacy of endoscopic axillary lymph node dissection combined with breast conserving surgery in the treatment of early breast cancer.Methods:The clinical data of 55 patients with early breast cancer were retrospectively analyzed.Of them,25 patients received simple excision of breast and axillary lymph node dissection(control group),and 30 patients underwent endoscopic axillary lymph node dissection combined with breast conserving surgery(observation group).The operative treatment,complications and prognosis of two groups were compared.Results:There was no significant difference in operation time,number of lymph node dissection,local recurrence rate,distant metastasis rate and mortality between the two groups(P>0.05).Compared with the control group,the observation group had lower intraoperative bleeding and complication rate,shorter hospital stay and higher excellent rate of breast aesthetics(P<0.05).Conclusion:The long-term effect of endoscopic axillary lymph node dissection combined with breast conserving surgery for early breast cancer is accurate,and the prognosis of breast cancer is similar to that of mastectomy.
