先天性心脏病患儿介入治疗的效果研究

摘要 目的:研究导管介入术治疗小儿先心病(CHD)的疗效及相较于传统外科手术的优势。方法:将某院收治的96例CHD患儿随机分为传统外科组和导管介入组各48例,传统外科组采用传统开胸术治疗,导管介入组采用导管介入术治疗,对比两组患儿的手术客观指标,于治疗前后分别检测心功能指标[左心室射血分数(LVEF)左心室收缩、舒张末期内径LVESD、LVEDD],并统计术后并发症的发生率。结果:导管介入组患儿手术耗用时间、术后机械通气时长、术后住ICU时间、总住院耗时均短于传统外科组(P<0.05),且术中失血量、胸腔引流量少于传统外科组(P<0.05);治疗后,导管介入组患儿LVEF高于传统外科组(P<0.05),LVESD、LVEDD低于传统外科组(P<0.05);导管介入组共8.33%的患儿术后发生并发症,低于传统外科组的22.92%(P<0.05)。结论:相较于传统开胸术,导管介入术治疗小儿CHD具有创伤小、康复快的优点,且对患儿心功能的改善效果更佳,术后并发症更少。 Objective:To study the efficacy of catheter intervention in the treatment of congenital heart disease(CHD)in children and its advantages over traditional surgery.Methods:96 children with CHD admitted to a hospital were randomly divided into traditional surgery group(n=48)and catheter intervention group(n=48),and the two groups were given traditional thoracotomy treatment and catheter interventional treatment,respectively.The surgical objective indexes of the two groups were compared,cardiac function indexes(LVEF,LVESD,LVEDD)were detected before and after treatment,and the incidence of postoperative complications was analyzed.Results:The operation time,postoperative mechanical ventilation duration,postoperative ICU stay time and total hospitalization time in the catheter intervention group were shorter than those in the traditional surgery group(P<0.05),and the intraoperative blood loss and thoracic drainage volume were less than those in the traditional surgery group(P<0.05).After treatment,LVEF in the catheter intervention group was higher than that in the traditional surgery group(P<0.05),while LVESD and LVEDD were lower than those in the traditional surgery group(P<0.05).A total of 8.33%of the children in the catheter intervention group had postoperative complications,which was lower than that in the traditional surgery group(22.92%,P<0.05).Conclusion:Compared with traditional thoracotomy,catheter intervention in the treatment of CHD in children has the advantages of less trauma and faster recovery,better improvement of heart function and fewer postoperative complications.
