动态心电图诊断老年睡眠呼吸暂停综合征的结果分析

Analysis of Results of Dynamic Electrocardiogram in the Diagnosis of Senile Sleep Apnea Syndrome

目的:探讨老年睡眠呼吸暂停综合征(SAS)的临床诊断中应用动态心电图的效果和临床价值。方法:选取86例老年睡眠障碍患者,均进行多导睡眠监测、动态心电图监测、动脉血氧检测。多导睡眠监测结果为判定的金标准,分析动态心电图监测对老年SAS的诊断价值,分析老年SAS患者和非老年SAS患者动态心电图和动脉血氧检测结果的差异性。结果:本研究中经多导睡眠监测阳性45例,阴性41例;动态心电图监测阳性41例,阴性45例。动态心电图诊断的敏感性为86.67%(39/45),特异性为95.12%(39/41)。老年SAS患者心电图异常及室性期前收缩的发生率明显高于非老年SAS(P<0.05),两组患者短阵室速、房室传导阻滞、窦性停搏的发生率无统计学差异(P>0.05);老年SAS患者AHI、TS90%、ODI高于非老年SAS,MSaO_(2)低于非老年SAS(P<0.05)。结论:老年SAS患者应用动态心电图诊断的诊断价值较高,对心脑血管疾病的防治意义重大。

Objective:To explore the effect and clinical value of dynamic electrocardiogram in the diagnosis of sleep apnea syndrome in the elderly.Methods:86 elderly patients with sleep disorders were selected and given polysomnography,dynamic electrocardiogram monitoring,and arterial blood oxygen detection.The results of polysomnography were the gold standard for judgment.The diagnostic value of dynamic electrocardiogram monitoring for elderly sleep apnea syndrome was analyzed,and the differences in the results of dynamic electrocardiogram and arterial blood oxygen detection between elderly sleep apnea syndrome patients and non-elderly sleep apnea syndrome patients were analyzed.Results:In this study,45 patients were positive and 41 patients were negative in polysomnography monitoring.41 patients were positive and 45 patients were negative in dynamic electrocardiogram monitoring.The sensitivity of dynamic electrocardiogram in the diagnosis of SAS was 86.67%(39/45)and the specificity was 95.12%(39/41).The incidence of abnormal ECG and premature ventricualr contraction in elderly patients with sleep apnea syndrome was significantly higher than that of non-elderly patients with sleep apnea syndrome(P<0.05).There was no statistically significant difference in the incidence of short array ventricular velocity,atrioventricular heart-block,and sinus arrest between the two groups(P>0.05).The AHI,TS90%,and ODI in elderly patients with SAS were higher than those in the non-elderly patients with SAS,while the MSaO_(2) was lower than that in the non-elderly patients with SAS(P<0.05).Conclusion:The diagnostic value of dynamic electrocardiogram in elderly patients with sleep apnea syndrome is high,which is of great significance to the prevention and treatment of cardiovascular and cerebrovascular diseases.
