产后PDF患者不同产后盆底康复治疗时机对盆底功能、尿失禁的影响

Effect of Different Timing of Postpartum Pelvic Floor Rehabilitation on Pelvic Floor Function and Urinary Incontinence in Patients with Postpartum PDF

目的:分析产后PDF患者不同产后盆底康复治疗时机对盆底功能、尿失禁的影响。方法:选取某院2019年12月~2020年12月诊治的92例产妇盆底功能障碍患者开展研究,随机将其均分为对照组和观察组各46例,对照组于产后12~18周展开产后盆底康复治疗,观察组于产后6~12周展开产后盆底康复治疗,比较两组康复效果。结果:治疗前两组的I类肌纤维肌力和Ⅱ类肌纤维肌力等级均基本一致(P>0.05),治疗后观察组的I类肌纤维肌力和Ⅱ类肌纤维肌力等级均明显高于对照组(P<0.05);与对照组躯体功能、心理功能和社会功能评分相比,观察组均明显偏高(P<0.05);与对照组性满意度评分、性态度评分、性交流评分和性反应评分相比,观察组均明显偏高(P<0.05);与对照组直肠脱垂发生率、子宫脱垂发生率、尿失禁发生率和膀胱脱垂发生率相比,观察组均明显偏低(P<0.05)。结论:给予产后PDF患者早期产后盆底康复治疗能够有效改善患者盆底肌功能,提升患者性功能和生活质量,降低并发症发生率,具有推广价值。

Objective:To analyze the influence of different postpartum pelvic floor rehabilitation treatment timing on pelvic floor function and urinary incontinence in postpartum PDF patients.Methods:92 cases of maternal pelvic floor dysfunction patients diagnosed and treated in a hospital from December 2019 to December 2020 were selected for this study.They were randomly divided into the control group(n=46)and the observation group(n=46).The control group received postpartum pelvic floor rehabilitation treatment at 12~18 weeks after delivery,and the observation group received postpartum pelvic floor rehabilitation treatment at 6~12 weeks after delivery,then the rehabilitation effect of the two groups was compared.Results:The type I myofiber muscle strength and Ⅱ myofiber muscle strength levels were almost the same in the two groups before the treatment(P>0.05).The type I myofiber muscle strength and Ⅱ myofiber muscle strength levels in the observation group after treatment were significantly higher than those in the control group(P<0.05).Compared with the control group,the scores of physical function,psychological function and social function in the observation group were significantly higher(P<0.05).Compared with the control group,the scores of sexual satisfaction,sexual attitude,sexual communication and sexual response in the observation group were significantly higher(P<0.05).Compared with the control group,the incidences of rectal prolapse,uterine prolapse,urinary incontinence and bladder prolapse were significantly lower in the observation group(P<0.05).Conclusion:Early postpartum pelvic floor rehabilitation therapy for postpartum PDF patients can effectively improve pelvic floor muscle function,enhance sexual function and quality of life of patients,and reduce the incidence of complications,which has the value of promotion.
