18F-FDG PET/CT技术评价T2DM对晚期结直肠癌患者PD-L1治疗疗效的影响 被引量:3

摘要 本研究探讨氟代脱氧葡萄糖正电子发射计算机断层显像/计算机断层显像(^(18)F-FDG PET/CT)技术评价2型糖尿病(T2DM)对晚期结直肠癌患者程序性死亡受体-1配体(PD-L1)治疗疗效的影响,选取104例晚期结直肠癌患者,根据是否伴有T2DM分为T2DM组(n=49)、无T2DM组(n=55)。比较两组疗效及18 F-FDG PET/CT技术参数:平均标准摄取值(SUV_(mean))、最大标准摄取值(SUV_(max))、代谢体积(MTV)、病灶糖酵解总量(TLG),探讨^(18)F-FDG PET/CT的应用价值。结果显示,T2DM组病理分期、疗效、SUV_(mean)、SUV_(max)、MTV、TLG与无T2DM组比较,差异有统计学意义(P<0.05);T2DM组、无T2DM组未缓解者SUV_(mean)、SUV_(max)、MTV、TLG均高于缓解者(P<0.05);T2DM组缓解者SUV_(mean)、SUV_(max)、MTV、TLG高于无T2DM组缓解者(P<0.05);SUV_(mean)、SUV_(max)、MTV、TLG与疗效呈负相关(P<0.05);SUV_(mean)、SUV_(max)、MTV联合TLG预测T2DM未缓解的价值最高。采用^(18)F-FDG PET/CT技术联合检测SUV_(mean)、SUV_(max)、MTV、TLG能评价T2DM对晚期结直肠癌患者PD-L1靶向治疗疗效的影响,并能客观、量化预测患者的治疗反应性。 This study explored the evaluation of ^(18)F-fluorodeoxyglucose positron emission computed tomography/computed tomography(^(18) F-FDG PET/CT)technology for the evaluation of type 2 diabetes(T2DM)on the programmed death receptor-1 ligand in patients with advanced colorectal cancer(PD-L1)The effect of treatment efficacy.A total of 104 patients with advanced colorectal cancer were selected,and they were divided into T2DM group(n=49)and no T2DM group(n=55)according to whether they were accompanied by T2DM.The efficacy and ^(18)F-FDG PET/CT technical parameters:average standard uptake value(SUV_(mean)),maximum standard uptake value(SUV_(max)),metabolic volume(MTV),total glycolysis of lesions(TLG)were compared between the two groups,and the application value of 18 F-FDG PET/CT were explored.The results showed that the pathological staging,curative effect,SUV_(mean),SUV_(max),MTV,TLG of the T2DM group were compared with those without T2DM,and the difference was statistically significant(P<0.05);T2DM and T2DM without remission were SUV_(mean),SUV_(max),MTV,TLG SUV_(mean),SUV_(max),MTV,and TLG in the T2DM group were higher than those in the non-T2DM group(P<0.05).SUV_(mean),SUV_(max),MTV,and TLG were negatively correlated with the efficacy(P<0.05).SUV_(mean),SUV_(max),MTV combined with TLG predict the highest value of T2DM unrelief.The combined detection of SUV_(mean),SUV_(max),MTV,and TLG with 18 F-FDG PET/CT technology can evaluate the effect of T2DM on the efficacy of PD-L1 targeted therapy in patients with advanced colorectal cancer,and can objectively and quantitatively predict the patient’s response to treatment.
