中药涂擦联合中药塌渍治疗股骨粗隆间骨折疼痛的疗效观察

Observation on the efficacy of traditional Chinese medicine inunctioncombined with traditional Chinese medicine hydrotherapeutics in the treatment of pain of intertrochanteric fracture of femur

目的探讨中药涂擦联合中药塌渍治疗股骨粗隆间骨折疼痛的疗效,为临床治疗股骨粗隆间骨折提供更多依据。方法选择2017年8月—2019年8月本院收治的80例股骨粗隆间骨折患者作为研究对象,按随机数表法分为对照组和观察组两组,每组各40例。对照组在常规药物治疗及护理基础上实施中药塌渍治疗,观察组在常规药物治疗及护理基础上实施中药涂擦联合中药塌渍治疗,10 d为1个疗程,两组患者均连续治疗2个疗程。比较两组患者治疗后的临床疗效、肿胀消退时间、负重行走时间、骨痂生成时间、骨折愈合时间、下地时间、疼痛评分、并发症发生情况以及治疗前后的髋关节功能评分。结果观察组临床疗效高于对照组(P<0.05),观察组患者肿胀消退时间、负重行走时间、骨痂生成时间、骨折愈合时间、下床时间均短于对照组(P<0.05),治疗前两组患者髋关节功能指标评分差异无统计学意义(P>0.05),治疗后两组患者髋关节功能指标评分均升高,观察组高于对照组(P<0.05);两组患者治疗前疼痛评分差异无统计学意义(P>0.05),治疗10 d后、治疗20 d后两组患者疼痛评分均下降,观察组低于对照组(P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论中药涂擦联合中药塌渍可有效改善股骨粗隆间骨折患者临床症状和髋关节功能,缩短骨折愈合时间和下地时间,降低疼痛水平和不良反应发生率,安全性较高。

Objective To explore the efficacy of Chinese medicine inunction combined with traditional Chinese medicine hydrotherapeuticsin the treatment of pain in intertrochanteric fracture of femur,and to provide more evidence for clinical treatment of intertrochanteric fracture of femur.Methods A total of 80 cases of patients suffered with intertrochanteric fracture,those were admitted to our hospital from August 2017 to August 2019,were enrolled in the study.According to random number table method they were divided into control group and observation group,40 cases in each group.Patients in control group received the treatment of traditional Chinese medicine hydrotherapeuticson the basis of conventional drug treatment and nursing implementation,and patients in observation group were given TCM inunction combined TCM hydrotherapeuticsbased on the conventional drug treatment and nursing implementation of traditional Chinese medicine.10 day was a course of treatment,and patients in both groups were treated for 2 courses continuously.The clinical efficacy,swelling regression time,walking on earth with load,callus formation time,fracture healing time,ground time,pain score,complications and hip function score before and after treatment were compared between the two groups.Results The clinical efficacy of the observation group was higher than that of the control group(P<0.05).The swelling regression time,weight-bearing walking time,callus formation time,fracture healing time and grounding time of the observation group were shorter than those of the control group(P<0.05).There was no significant difference in hip function index scores between the two groups before treatment(P>0.05).After treatment,the hip function index scores of the two groups increased,and it was higher in the observation group than in the control group(P<0.05).There was no significant difference in pain scores between the two groups before treatment(P>0.05).The pain scores of the two groups decreased after 10 days and 20 days of treatment,and the pain scores of the observation group were lower than those of the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusions TCM inunction combined with TCM hydrotherapeutics could effectively improve the clinical symptoms and hip function of patients with intertrochanteric fracture of femur,shorten the fracture healing time and the time to ground,reduce the pain level and the incidence of adverse reactions,with high safety.
