Application Effects of Self-developed Monitoring System for Rational Use of Drugs in Inpatients Orders

目的:探索住院医嘱合理用药监测系统的应用及管理在提高住院患者用药医嘱合理性水平中的成效,提高合理用药水平。方法:将2019年4月到2021年3月期间住院医嘱合理用药监测系统拦截不合理医嘱信息导入Excel工作表,并对监测结果进行分析。结果:住院医嘱合格率由2020年3月(上线前)的77.55%上升到2021年3月(上线后)的98.02%,系统上线后住院医嘱合格率每个季度分别为79.07%、82.45%、94.17%、98.28%;通过SPSS20.0统计软件对药师端进行无效回复医嘱通报前后相同季度数据进行χ^(2)检验,发现2020年第二季度与2021年第二季度返回修改率和无效回复率均存在显著统计学差异(P<0.01)。结论:住院医嘱合理用药监测系统运行期间,提高了本院的合理用药和用药管理水平。系统拦截规则灵活,自定义设置较为全面,有效阻止了可能存在的药物安全隐患。但是,系统还需进一步维护升级,以适应更高标准的用药要求。

Objective: To explore the effectiveness of the application and management of the monitoring system of rational drug use in improving the rationality of medical advice for inpatients so as to improve the level of rational drug use. Methods: The unreasonable medical information intercepted by the monitoring system of rational drug use in inpatient medical orders from April, 2019 to March, 2021 was imported into the excel worksheet, and the monitoring results were analyzed. Results: The qualified rate of inpatient medical orders increased from 77.55% in March, 2020(before the launch of the system) to 98.02% in March, 2021(after the launch of the system), and the qualified rates of inpatient medical orders were 79.07%, 82.45%, 94.17% and 98.28%, respectively, every quarter after the rational drug use monitoring system was launched. χ^(2)test was conducted on the data of the same quarter before and after the notification of invalid reply to medical orders by SPSS 20.0 statistical software, and it was found that there were significant statistical differences in the rate of return modification and invalid response between the second quarter of 2020 and the same quarter of 2021(P<0.01). Conclusion: During the operation of the system, the level of rational drug use and the drug administration in our hospital have been improved. The system interception rules were set up flexibly and autonomously,effectively preventing potential drug safety risks. The system needs to be further maintained and upgraded so as to meet higher standards of drug requirements.
