摘要
目的:探讨闭合复位驱血带临时固定交锁髓内钉固定治疗胫骨骨折的临床效果。方法:回顾性分析广州医科大学附属第四医院骨科2010年9月-2020年9月采用闭合复位驱血带临时固定交髓内钉治疗70例胫骨骨折患者的临床资料。结果:平均手术时间45 min(30-70 min),复查X片骨折复位良好,随访时间平均18个月(12-24个月),骨折愈合时间平均6个月(3-9个月),按Johner-Wruhs评定标准,优61例,良7例,中2例,优良率97.1%,无骨不连或延迟愈合。1例术后1周余出现骨折处肿胀积血,行切开引流后治愈,12例出现膝前痛,术后1月消失。结论:胫骨骨折闭合复位驱血带捆绑使复位容易维持,且取材简单,操作容易,建议推广应用。
Objective:To investigate the clinical effect of tibial fracture treated by closed reduction and temporary fixation with blood drive belt and interlocking intramedullary nail.Methods:The clinical data of70 patients with tibial fracture treated by closed reduction and temporary fixation with blood drive belt and interlocking intramedullary nail were retrospectively analyzed from Sep 2010 to Sep 2020 inthe hospital.Results:The average operation time was 45 minutes(30-70 minutes),and X-ray examination results showed satisfactory postoperative reduction.The mean follow-up time was 18 months(12-24 months),and the mean fracture healing time was 6 months(3-9 months).According to JohnerWruhs standard,61 cases were excellent,7 cases were good and 2 cases were medium.The excellent and good rate was97.1%.No bone nonunion or delayed union.1 case developed swelling and blood accumulation at the fracture more than one week after operation,which was cured after incision and drainage,and 12 cases developed anterior knee pain,which disappeared one month after operation.Conclusion:Clinical applications have shown that closed reduction of tibial fractures is tied with blood drive belt,which makes the reduction easy to maintain,simple to obtain materials,easy to operate,and suitable for promotion.
