感觉统合训练结合吞咽康复干预对喉部分切除患者术后吞咽功能和生命质量的影响

Effects of sensory integration training combined with swallowing rehabilitation nursing measures on the swallowing function and life quality of patients after partial laryngectomy

目的探讨感觉统合训练结合吞咽康复护理措施对喉部分切除患者术后的影响。方法选取2019年1月至2020年6月行喉部分切除术的70例患者作为研究对象,按照随机数字表法将其分为对照组和研究组,每组35例。对照组患者术后给予常规护理,研究组患者在此基础上给予感觉统合训练结合吞咽康复干预措施。采用洼田饮水试验,中文版安德森吞咽困难量表(M.D.anderson dysphagia inventory,MDADI),中国头颈部癌患者生命质量测量量表(quality of life instruments for cancer patients-head and neck cancer,QLICP-HN)评估患者干预前后吞咽功能及生命质量的变化。结果干预前2组患者洼田饮水试验评级、MDADI量表总分及各维度评分QLICP-HN各维度评分比较,差异无统计学意义(P>0.05);干预后,研究组患者洼田饮水试验评级优于对照组患者,MDADI量表总体、情绪、生理功能维度得分及总分高于对照组患者,QLICP-HN量表中患者躯体功能、心理功能、共性症状及不良反应维度得分高于对照组患者,差异均有统计学意义(P<0.05)。结论对于喉部分切除术患者,采用感觉统合训练及吞咽康复干预,可有效改善患者术后吞咽功能及生命质量,具有一定的临床价值。

Objective To investigate the effects of sensory integration training combined with swallowing rehabilitation nursing measures on the swallowing function and life quality of patients after partial laryngectomy.Methods A total of 70 patients who underwent partial laryngectomy in our hospital from January 2019 to June 2020 were enrolled in the study,who were randomly divided into control group and research group,with 35 patients in each group.The patients in control group were treated by routine nursing after surgery,however,thepatients in research group,on the basis of control group,were treated by sensory integration training combined with swallowing rehabilitation interventions.The changes of swallowing function and life quality of patients before and after intervention were assessed by water swallow test,MD Anderson Dysphagia Inventory(MDADI),and Quality of life instruments for cancer patients-head and neck cancer(QLICP-HN).Results Before intervention,there were no significant differences in the water swallow test rating,the MDADI scale total scores and the QLICP-HN scores between the two groups(P>0.05).After intervention,the water swallow test rating in research group was superior to that in control group,and the emotional,and physiological function dimension scores and total scores of MDADI scale in research group were significantly higher than those in control group(P<0.05).Moreover the scores of physical function,mental function,common symptoms and adverse reaction dimensions according to QLICP-HN scale in research group were significantly higher than those in control group(P<0.05).Conclusion The sensory integration training combined with swallowing rehabilitation nursing measures for the patients undergoing partial laryngectomy can effectively improve the postoperative swallowing function and life quality of patients,which has certain clinical application value.
