差异性血液净化方式对血清铁调素的影响

摘要 目的研究差异性血液净化方式对血清铁调素的变化影响。方法血液净化治疗患者120例按照随机抽签法等分成联合组及常规组,每组各60例。常规组实施维持性血液透析治疗,联合组则予以血液透析联合血液透析滤过治疗。2组均进行为期3个月的治疗,比较干预前后2组血清铁调素及相关指标水平变化情况,炎性因子水平[白介素-6(IL-6)、超敏C-反应蛋白(h-CRP)]变化情况,脂肪因子水平(脂联素、抗抵素、瘦素)变化情况。并作相关性分析。结果治疗后联合组血清铁调素水平低于常规组,而血红蛋白水平高于常规组(P<0.05)。治疗后联合组血清IL-6、CRP水平均低于常规组(P<0.05)。治疗后联合组脂联素水平高于常规组,而抵抗素、瘦素水平均低于常规组(P<0.05)。经Pearson相关性分析可得:血清铁调素与IL-6、CRP均呈正相关(r值分别为0.421、0.433,P<0.05)。结论血液透析联合血液透析滤过相较于单纯血液透析可有效下调血清铁调素水平,同时有利于减轻微炎性反应,改善脂肪因子表达水平,值得临床推广应用。 Objective To investigate the effects of differential blood purification modes on the changes of serum Hepcidin.Methods A total of 120 patients who received blood purification treatment in our hospital from December 2017 to June 2019 were enrolled in the study,who were randomly divided into combination treatment group and conventional treatment group(control group),with 60 cases in each group.The patients in conventional treatment group received maintenance hemodialysis treatment,however,the patients in combination treatment group received hemodialysis combined hemodialysis filtration treatment,with a treatment course of 3 months for both groups.The levels of serum Hepcidin and related indicators,inflammatory factors and adipokine were observed and compared between the two groups.Results After treatment the serum levels of Hepcidin in combination treatment group were significantly lower than those in control group,however,the hemoglobin levels were significantly higher than those in control group(P<0.05).And the serum levels of IL-6 and CRP in combination treatment group were significantly lower than those in control group(P<0.05).After treatment the adiponectin levels in combination treatment group were significantly higher than those in control group,while resistin and leptin levels were significantly lower than those in control group(P<0.05).Pearson correlation analysis showed that there was a positive correlation between serum Hepcidinand IL-6 as well as CRP(P<0.05).Conclusion Compared with simple hemodialysis alone,the hemodialysis combined with hemodiafiltration can effectively reduce the levels of Hepcidin,relieve microinflammatory response and improve adipokine expression,thus,it is worthy of clinical application and promotion.
