不同剂量康柏西普治疗早产儿视网膜病变短期效果观察

Observation on the short-term efficacy of different doses of conbercept in the treatment of retinopathy of prematurity

目的观察不同剂量康柏西普玻璃体内注射治疗早产儿视网膜病变(ROP)的短期效果。方法前瞻性随机对照研究。纳入2018年1月至2020年6月在郑州大学附属儿童医院确诊为双眼ROP且需要行玻璃体内注射抗血管内皮生长因子(VEGF)治疗的患儿80例,根据随机数字表法分为两组:对照组,39例,玻璃体内注射0.25 mg/0.025 ml康柏西普;观察组,41例,玻璃体内注射0.15 mg/0.015 ml康柏西普,观察比较两组短期疗效。结果两组术后治愈率差异无统计学意义(χ^(2)=0.65,P=0.420),两组间术中术后并发症发生率差异也无统计学意义(χ^(2)=0.27,P=0.603)。两组治疗后短期各时间点眼压较术前升高,不同时间点眼压差异有统计意义(F=17.67,P<0.05),观察组注射后5 min及30 min的眼压低于对照组(F=56.87,P<0.05)。结论对早产儿视网膜病变进行两种剂量的康柏西普玻璃体内注射,均可有效抑制ROP病情进展,达到满意的治愈率。而玻璃体内注射0.15 mg/0.015 ml剂量康柏西普,术后眼压的短期变化更小。

Objective To observe the short-term efficacy of different doses of intravitreal injection of conbercept in the treatment of retinopathy of prematurity(ROP).Methods This was a prospective randomized control study.Total of 80 cases with binocular ROP from Jan.2018 to Jun.2020 in Children’s Hospital Affiliated to Zhengzhou University were included.They were divided into two groups based on the random number table method.The 39 cases of control group received intravitreal injection of 0.25 mg/0.025 ml conbercept;the 41 cases of observation group received intravitreal injection of 0.15 mg/0.015 ml conbercept.The short-term efficacy were observed and compared between the two groups.Results There was no statistically significant difference in postoperative cure rate between the two groups(χ^(2)=0.65,P=0.420).There was no significant difference in the incidence of intraoperative and postoperative complications between the two groups(χ^(2)=0.27,P=0.603).The intraocular pressure(IOP)of the two groups at different time points after treatment were higher than that before surgery,and the difference at different time points was statistically significant(F=17.67,P<0.05).The IOP in the observation group in 5 min and 30 min after injection was lower than that in the control group(F=56.87,P<0.05).Conclusion Both two doses of conbercept can inhibit the disease progression of ROP and achieve satisfactory cure rate.However,intravitreal injection of 0.15 mg/0.015 ml of conbercept bring less intraocular pressure changes in the short term.
