超声乳化联合玻璃体切除术中晶状体后囊切开术预防后发性白内障

Posterior capsulotomy during phacoemulsification combined with vitrectomy for preventing after cataract

目的评价超声乳化人工晶状体植入联合玻璃体切除术中晶状体后囊切开术预防后发性白内障的效果。方法前瞻性随机对照研究。将郴州市第一人民医院2016年10月至2018年10月老年性白内障伴发玻璃体视网膜疾病80例(80眼)按随机数字表法分为两组,试验组40眼行白内障超声乳化人工晶状体植入联合23G玻璃体切除术,术中行一期晶状体后囊切开术;对照组40眼相同手术但术中未行后囊切开术。术后随访24个月,比较两组的效果。结果随访期末,试验组术后未发生后发性白内障;对照组12眼(30.00%,12/40)发生后发性白内障(P<0.001)。两组术后最佳矫正视力均较术前明显提高(P<0.001)。结论一期晶状体后囊切开术预防后发性白内障效果良好。

Objective To evaluate the efficacy of primary posterior capsulotomy during phacoemulsification and intraocular lens implantation combined with vitrectomy in the prevention of after cataract.Methods This was a prospective randomized controlled study.Eighty eyes of 80 cases of patients with age-related cataract and vitreoretinopathy in the First People’s Hospital of Chenzhou District from Oct.2016 to Oct.2018 were divided into two groups based on random number table methods.There were 40 eyes of the trial group received posterior capsulotomy during phacoemulsification combined with vitrectomy,and 40 eyes of the control group without posterior capsulotomy.The patients were followed up for 24 months.The efficacy between the two groups were compared.Results At the end of the follow-up,no after cataract occurred in the trial group;after cataract developed on 12 eyes(30.00%,12/40)in the control group(P<0.001).The best corrected visual acuity in both groups was significantly higher than that before operation(P<0.001).Conclusion Primary posterior capsulotomy during phacoemulsification combined with vitrectomy is effective in the prevention of after cataract.
