钛夹标记在局部晚期胃癌腹腔镜根治术患者中的应用效果

Application effect of titanium-based marker clip in patients with locally advanced gastric cancer undergoing laparoscopic radical resection

目的探讨钛夹标记在局部晚期胃癌腹腔镜根治术患者中的应用效果。方法依据手术方式将230例局部晚期胃癌患者分为腹腔镜组和联合组,每组115例,腹腔镜组患者给予腹腔镜胃癌根治术治疗,联合组患者给予腹腔镜胃癌根治术联合钛夹标记。比较两组患者的手术相关指标、生活质量[消化系统疾病生存质量指数量表(GLQI)]、自主活动时间、疼痛程度[视觉模拟评分法(VAS)]、镇痛剂使用量和并发症发生情况。结果联合组患者术中出血量明显少于腹腔镜组(P﹤0.01),首次进食时间、住院时间均明显短于腹腔镜组(P﹤0.01)。术后2周、2个月,联合组患者GLQI量表评分均明显高于腹腔镜组(P﹤0.01),自主活动时间明显长于腹腔镜组患者(P﹤0.01)。术后1、5天,联合组患者VAS评分均明显低于腹腔镜组(P﹤0.01),镇痛剂使用量明显低于腹腔镜组患者(P﹤0.01)。联合组并发症总发生率为3.48%,低于腹腔镜组患者的11.30%(P﹤0.05)。结论腹腔镜胃癌根治术联合钛夹标记可以有效改善局部晚期胃癌患者的手术相关指标、生活质量,降低镇痛剂使用量和并发症发生率。

Objective To investigate the application effect of titanium-based marker clip in patients with locally advanced gastric cancer undergoing laparoscopic radical resection.Method A total of 230 patients with locally advanced gastric cancer were divided into laparoscopic group and combined group according to the surgical approaches,with 115 cases in each group.The patients in the laparoscopic group were treated with laparoscopic radical gastrectomy for gastric cancer,and the patients in the combined group were treated with laparoscopic radical gastrectomy combined with the titanium-based marker clip.The operation-related indicators,quality of life[gastrointestinal quality of life index(GLQI)],spontaneous activity time,pain level[visual analog scale(VAS)],analgesic dosage,and complications were compared between the two groups.Result The intraoperative blood loss volume in the combined group was significantly less than that in the laparoscopic group(P<0.01),and the time to first feeding and the duration of hospital stay were significantly shorter than those in the laparoscopic group(P<0.01).At 2 weeks and 2 months after the operation,the GLQI score of the combined group was significantly higher than that of the laparoscopic group(P<0.01),and the spontaneous activity time was significantly longer than that of the laparoscopic group(P<0.01).At 1 and 5 days after the operation,the VAS scores of the combined group were significantly lower than those of the laparoscopic group(P<0.01),and the dosage of analgesics used was significantly lower than that of the laparoscopic group(P<0.01).The total incidence of complications in the combined group was 3.48%,which was lower than 11.30%in the laparoscopic group(P<0.05).Conclusion Laparoscopic radical gastrectomy combined with titanium-based marker clip could effectively improve patients’surgical-related indicators,quality of life,and reduce the use of analgesics and the incidence of complications.
