复胃消萎汤治疗脾虚湿热型慢性萎缩性胃炎伴异型增生临床观察

Clinical observation of Fuwei Xiaowei decoction on chronic atrophic gastritis with dysplasia and syndrome of spleen deficiency and damp-heat

目的:观察复胃消萎汤对脾虚湿热型慢性萎缩性胃炎患者的临床疗效。方法:收集脾虚湿热型慢性萎缩性胃炎患者70例,随机分为两组各35例。对照组予西药对症支持治疗,治疗组在对照组基础上加用复胃消萎汤浓缩颗粒,治疗8周后观察两组患者胃黏膜病理改变、临床证候积分、临床总有效率及血清中胃泌素-17 (G-17)、胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)、PGR值的变化。结果:治疗8周后,治疗组炎症反应、腺体萎缩、异型增生评分均明显低于对照组(P﹤0.05);治疗组临床证候积分明显降低,且优于对照组(P﹤0.05);对照组与治疗组的临床总有效率分别为74.29%和88.57%,两组比较具有统计学差异(P﹤0.05);治疗组血清中G-17、PGⅠ、PGⅡ水平及PGR值均明显高于对照组(P﹤0.05)。结论:复胃消萎汤联合西药治疗CAG在改善患者胃黏膜病变、缓解临床症状方面疗效确切,并且明显优于单独使用西药,在提高G-17、PGⅠ、PGⅡ水平,抑制病理进展方面也有明显优势。

Objective:To observe the effects of Fuwei Xiaowei decoction on patients with chronic atrophic gastritis(CAG)of spleen deficiency and damp-heat type.Methods:70 cases of collected patients with chronic atrophic gastritis of spleen deficiency damp-heat type were randomly divided into two groups,35 cases in each group.The control group was symptomatically supportively treated with western medicine,while the treatment group was treated plus with concentrated granule of Fuwei Xiaowei decoction on the basis of control group.After 8 weeks of treatment,the pathological change of gastric mucosa and the changes of clinical symptom score,clinical total effective rate as well as gastrin-17(G-17),pepsinogen Ⅰ(PG Ⅰ),pepsinogen Ⅱ(PG Ⅱ)and PGR value in serum of patients were observed in two groups.Results:After 8 weeks of treatment,the scores of inflammatory reaction,gland atrophy and dysplasia in the treatment group were significantly lower than those in the control group(P﹤0.05);the score of clinical symptoms of treatment group was significantly lower than that of control group(P﹤0.05);the clinical total effective rates of control group and treatment group were 74.29% and 88.57% respectively,and the comparative difference between two groups was statistically significant(P ﹤ 0.05);the levels of G-17,PG Ⅰ and PG Ⅱ as well as the value of PGR in serum of treatment group were significantly higher than those of control group(P﹤0.05).Conclusion:Fuwei Xiaowei decoction combined with western medicine for treating CAG has the definite efficacy in improving gastric mucosal lesions of patients and relieving clinical symptoms,is significantly better than simple western medicine,and has the obvious effects in improvement of levels of G-17,PG Ⅰ and PG Ⅱ as well as in inhibition of pathological progression.
