腹部增强多排螺旋CT增强检查联合血清CEA、CA199、CA72-4、CYFRA21-1检测在右半结肠癌诊断中的应用价值

Application Value of Abdominal enhanced Multi-slice Spiral CT Combined with Serum CEA,CA199,CA72-4 and CYFRA21-1 in the Diagnosis of Right Hemicolon Cancer

为探讨腹部增强多排螺旋CT增强检查联合血清癌胚抗原(CEA)、糖链抗原199(CA199)、糖链抗原72-4(CA72-4)、细胞角蛋白19片段(CYFRA21-1)检测在右半结肠癌诊断中的应用价值,随机选择我院2018年6月至2020年5月收诊的右半结肠癌患者70例,将其纳入观察组,另选择同期体检健康的受试者70名,将其纳入对照组。采用腹部增强多排螺旋CT增强检查,检测血清CEA、CA199、CA72-4、CYFRA21-1水平,比较2组受试者血清CEA、CA199、CA72-4、CYFRA21-1水平和CT评分。分析不同级别CT评分患者的血清CEA、CA199、CA72-4、CYFRA21-1阳性情况,判断腹部增强多排螺旋CT增强检查联合血清CEA、CA199、CA72-4、CYFRA21-1检测的诊断价值。结果显示,观察组患者血清CEA、CA199、CA72-4、CYFRA21-1水平和CT评分均高于对照组(P <0.05),低级别CT评分患者血清CEA、CA199、CA72-4、CYFRA21-1阳性率低于高级别(P <0.05),联合诊断具有较高的诊断价值(P <0.05)。结果表明,腹部增强多排螺旋CT增强检查联合血清CEA、CA199、CA72-4、CYFRA21-1检测能提高右半结肠癌的诊断价值,建议使用。

To investigate the value of abdominal enhanced multi-slice spiral CT combined with serum carcinoembryonic antigen(CEA),sugar chain antigen 199(CA199),sugar chain antigen 72-4(CA72-4)and cytokeratin 19 fragment(CYFRA21-1)in the diagnosis of right hemicolon cancer,70 patients with right hemicolon cancer admitted in author’s hospital from June 2018 to May 2020 were randomly selected and included in the observation group,in the same period,70 healthy subjects were selected and included in the control group.The levels of serum CEA,CA199,CA72-4 and CYFRA21-1 were detected by abdominal enhanced multi-slice spiral CT;the levels of serum CEA,CA199,CA72-4 and CYFRA21-1 and CT scores were compared between the two groups;the positive status of serum CEA,CA199,CA72-4 and CYFRA21-1 in patients with different CT scores were analyzed,and the diagnostic value of abdominal enhanced multislice spiral CT combined with serum CEA,CA199,CA72-4 and CYFRA21-1 were judged.As results,the levels of serum CEA,CA199,CA72-4 and CYFRA21-1 and CT scores in the observation group were higher than those in the control group(P <0.05).The positive rates of serum CEA,CA199,CA72-4 and CYFRA21-1 in patients with low-level CT scores were lower than those in patients with high-level CT scores(P <0.05).Combined diagnosis has high diagnostic value(P <0.05).The results show that abdominal enhanced multi-slice spiral CT combined with serum CEA,CA199,CA72-4 and CYFRA21-1 could improve the diagnostic value of right hemicolon cancer,which is recommended.
