舌形超薄皮瓣技术治疗耳廓瘢痕疙瘩的临床价值

Clinical value of tongue shaped ultra-thin flap technique in the treatment of auricular keloid

目的了解舌形超薄皮瓣技术治疗耳廓瘢痕疙瘩的临床价值。方法抽取2011年1月至2019年12月于许昌市立医院采用舌形超薄皮瓣技术治疗耳廓瘢痕疙瘩患者57例并设为观察组,另抽取同期采用单纯手术切除术治疗者83例设为对照组。收集两组患者的性别、年龄、病程、家族史、皮损发生诱因、皮损形状大小、皮损累及部位与范围、治疗期间及治疗后6个月治疗相关不良事件、治疗后6个月随访信息等数据。对所有数据进行统计学分析。结果两组患者性别、年龄、病程、发病诱因、家族史、皮损形状、皮损长径及皮损累及部位等一般资料比较差异未见统计学意义(P>0.05)。两组患者合并感染、皮损加重及局部血肿等治疗相关不良事件发生率比较差异未见统计学意义(P>0.05)。治疗后,观察组皮损改善率(100.00%,57/57)高于对照组(73.49%,61/83),P<0.05;皮损改善后复发率(15.79%,9/57)低于对照组(39.34%,24/83),P<0.05;治疗满意度评分[(7.54±1.22)分]高于对照组[(5.49±1.17)分],P<0.05。结论舌形超薄皮瓣技术治疗耳廓瘢痕疙瘩有效率高、复发率低、患者满意度高且安全性好,是一种操作简单、有效、实用的治疗方法。

Objective To learn the clinical value of tongue shaped ultra-thin flap technique in the treatment of auricular keloid.Methods Fifty-seven patient with auricular keloid treated by tongue-shaped ultra-thin skin flap technique in Xuchang Municipal Hospital from January 2011 to December 2019 were collected as observation group,while 83 patients treated by surgical resection during the same period were collected as control group.The gender,age,course of disease,family history,inducement of skin lesions,shape and size of skin lesions,location and extent of skin lesions,treatment-related adverse events during treatment and within 6 months after treatment,6-month follow-up information after treatment of the two groups were collected.All data were statistically analyzed.Results There was no significant difference in general information between the two groups,such as gender,age,course of disease,inducement factors,family history,shape of skin lesions,maximum diameter of skin lesions,and location of skin lesions(P>0.05).There was no difference in the incidence of treatment-related adverse events such as infection,exacerbation of skin lesions and local hematoma between two groups(P>0.05).After treatment,the improvement rate of skin lesions in the observation group(100.00%,57/57)was higher than that in the control group(73.49%,61/83),P<0.05;the recurrence rate of observation group(15.79%,9/57)after improvement was lower than that of control group(39.34%,24/83),P<0.05;the patients’treatment satisfaction score of observation group(7.54±1.22)was higher than that of control group(5.49±1.17),P<0.05.Conclusions The tongue-shaped ultra-thin skin flap technique is a simple,effective and practical treatment method for auricle keloid,which has a high effective rate,low recurrence rate,high patient satisfaction and good safety.
