外剥内扎术联合中药熏洗治疗环型混合痔临床研究

Clinical Study of Milligan-Morgan Hemorrhoidectomy Combined with Chinese Medicine Fuming-Washing Therapy for Circular Mixed Hemorrhoids

目的:观察外剥内扎术联合中药熏洗治疗环型混合痔对机体恢复情况及疼痛缓解情况的影响。方法:将60例环型混合痔患者按随机数字表法分为观察组与对照组各30例,对照组采取外剥内扎术治疗,观察组在对照组基础上采用中药熏洗治疗,治疗后比较2组临床效果。结果:观察组总有效率93.33%,高于对照组73.33%(P<0.05)。与同组术后1 d比较,术后7 d2组视觉模拟评分法(VAS)评分、水肿评分均降低(P<0.05),肛管静息压、肛管舒张压及肛管最大收缩压均升高(P<0.05),肛管高压带长度缩短(P<0.05);与对照组术后7 d比较,观察组术后7 d VAS评分、水肿评分较低(P<0.05),肛管静息压、肛管舒张压及肛管最大收缩压均较高(P<0.05),肛管高压带长度较短(P<0.05)。观察组并发症发生率10.00%,低于对照组33.33%(P<0.05)。观察组住院时间、创面愈合时间均短于对照组(P<0.05)。结论:采取外剥内扎术联合中药熏洗治疗环型混合痔与单纯采取外剥内扎术比较,有利于提升临床效果,减轻疼痛及水肿程度,改善肛门功能,降低并发症的发生,并可缩短住院治疗时间,促进创面愈合。

Objective:To observe the effect of milligan-morgan hemorrhoidectomy combined with Chinese medicine fuming-washing therapy on body recovery and remission of pain in patients with circular mixed hemorrhoids.Methods:A total of 60 cases of patients with circular mixed hemorrhoids were divided into the observation group and the control group according to the random number table method,with 30 cases in each group.The control group was treated with milliganmorgan hemorrhoidectomy,and the observation group was additionally treated with Chinese medicine fuming-washing therapy based on the treatment of the control group.Clinical effects after treatment were compared between the two groups.Results:The total effective rate was 93.33%in the observation group,higher than that of 73.33%in the control group(P<0.05).On the 7 thday after the surgery,scores of Visual Analogue Scale(VAS)and edema in the two groups were decreased when compared with those on the 1 stday after the surgery(P<0.05);anal resting pressure,anal diastolic pressure and anal maximum systolic pressure were increased(P<0.05),and the length of high pressure zone was shortened(P<0.05).On the7 thday after surgery,when compared with those in the control group,scores of VAS and edema in the observation group were lower(P<0.05),resting pressure,anal diastolic pressure and anal maximum systolic pressure were higher(P<0.05),and the length of high pressure zone was shortened(P<0.05).The incidence of complications was 10.00%in the observation group,lower than that of 33.33%in the control group(P<0.05).The hospitalization time and the wound healing time in the observation group were shorter than those in the control group(P<0.05).Conclusion:When compared with the simple application of milligan-morgan hemorrhoidectomy,the combined treatment with Chinese medicine fuming-washing therapy for circular mixed hemorrhoids is helpful to enhance clinical effect,relieve pain and edema degree,improve anal function,reduce the incidence of complications and hospitalization time,and promote wound healing.
