火龙灸联合腕踝针治疗寒湿痹阻型腰椎间盘突出症临床研究

Clinical Study on Huolong Moxibustion Combined with Wrist-Ankle Acupuncture for Lumbar Disc Herniation of Cold-Dampness Obstruction Type

目的:观察火龙灸联合腕踝针治疗寒湿痹阻型腰椎间盘突出症(LDH)的临床疗效。方法:选取82例寒湿痹阻型LDH患者,按随机数字表法分为观察组及对照组各41例,对照组给予腰椎牵引联合功能锻炼治疗,观察组在对照组治疗方案基础上给予火龙灸联合腕踝针治疗,2组均治疗2周。观察2组临床疗效,比较2组治疗前后中医证候评分、血清炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)与白细胞介素-1β(IL-1β)]水平、疼痛视觉模拟评分法(VAS)评分以及Oswestry功能障碍指数(ODI)评分的变化。结果:观察组临床疗效总有效率为97.56%,高于对照组80.49%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组腰痛、烦躁易怒、倦怠乏力、腿痛中医证候评分均较治疗前降低,观察组上述4项评分均低于对照组,差异均有统计学意义(P<0.05)。治疗2周后,2组TNF-α、IL-6、IL-1β水平均较治疗前降低,观察组上述3项水平均低于对照组,差异均有统计学意义(P<0.05)。治疗后,2组VAS及ODI评分均较治疗前下降,观察组上述2项评分均低于对照组,差异均有统计学意义(P<0.05)。结论:火龙灸联合腕踝针治疗寒湿痹阻型LDH疗效确切,可缓解临床症状,提高临床疗效,改善腰椎功能,减轻疼痛程度。

Objective:To observe the clinical effect of the therapy of Huolong moxibustion combined with wrist-ankle acupuncture on lumbar disc herniation(LDH)of cold-dampness obstruction type.Methods:A total of 82 patients with LDH of cold-dampness obstruction type were selected and divided into the observation group and the control group according to the random number table method,with 41 cases in each group.The control group was treated with lumbar vertebrae traction combined with functional exercise,and the observation group was additionally treated with Huolong moxibustion combined with wrist-ankle acupuncture based on the treatment of the control group.Both groups were treated for two weeks.The clinical effects in the two groups were observed.Before and after treatment,the changes of levels of Chinese medicine syndrome scores and serum inflammatory factors,including tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-1β(IL-1β)and the scores of Visual Analogue Scale(VAS)and Oswestry Disability Index(ODI)in the two groups were compared.Results:The total clinical effective rate was 97.56%in the observation group,and was 80.49%in the control group,the difference being significant(P<0.05).After treatment,the Chinese medicine syndrome scores of low back pain,irritability,fatigue and leg pain in the two groups were decreased when compared with those before treatment,and the above four scores in the observation group were lower than those in the control group,differences being significant(P<0.05).After two weeks of treatment,the levels of TNF-α,IL-6 and IL-1βin both groups were decreased when compared with those before treatment,and the above three levels in the observation group were lower than those in the control group,differences being significant(P<0.05).After treatment,the scores of VAS and ODI in the two groups were decreased when compared with those before treatment,and the above two scores in the observation group were lower than those in the control group,differences being significant(P<0.05).Conclusion:The therapy of Huolong moxibustion combined with wristankle acupuncture has a definite curative effect on LDH of cold-dampness obstruction type,which can relieve the clinical symptoms,improve the clinical effects,enhance the lumbar function,and reduce the degree of pain.
